不同术式对胃癌穿孔患者疗效的影响评价

目的探讨不同术式对胃癌穿孔治疗的临床疗效。方法 2006年9月～2009年9月期间,某院诊治的90例胃癌穿孔患者,根据术式分为穿孔修补术组、姑息性切除术组和根治性切除术组,每组各30例,对比其手术疗效。结果术后随访2～5年。与穿孔修补术相比,姑息性切除和根治性切除术的死亡率和并发症明显降低,P﹤0.05,差异有统计学意义;与穿孔修补术组相比,姑息性切除术组和根治性切除术组的生存期明显延长,P﹤0.05,差异有统计学意义;与姑息性切除术相比(580.3±35.1)d,根治性切除术组的生存期明显延长(770.6±46.2)d,P﹤0.05,差异有统计学意义。结论姑息性切除术和根治性切除术,都可以明显延长胃癌穿孔患者的生存期,根治性切除术是唯一可能治愈胃癌的手术方式。

Evaluation on the Different types of operation on the patients with perforated gastric cancer curative effect evaluation

OBJECTIVE To explore the different types of operation on the clinical efficacy of the treatment of perforated gastric cancer.METHODS From Sep.2006 to Sep.2009 during the month,90 patients with perforated gastric cancer were selected in our hospital,and they were divided into perforation repair group,palliative resection group and radical resection group,30 cases in each group.We compared the operation effect.RESULTS The patients were followed up for 2-5 years.Compared with perforation,the mortality and complications of palliative resection and radical resection significantly decreased(P﹤0.05).When compared with the perforation repair group,the survival period of palliative resection group and radical excision group was significantly prolonged(P﹤0.05).When compared with the palliative resection(580.3 ± 35.1),the survival rate of the radical excision group was significantly prolonged(770.6 ± 46.2),P﹤0.05.CONCLUSION Palliative resection and radical excision can significantly prolong the survival time of patients with perforated gastric cancer,radical resection may be the only potentially curable gastric cancer operation.
