空间-时间相关成像技术在胎儿大血管成像的应用

摘    要:目的评价空间一时间相关成像(spatiotemporal image correlation,STIC)技术在胎儿大血管中的应用。方法使用PHILIPSIU-22彩色多普勒超声仪对245例单胎妊娠20~38孕周的胎儿进行进行二维心脏扫查后行空间一时间相关成像扫查,对所获得容积数据库进行分析。结果 245例胎儿均获得容积数据显示的三维平面图像(即A、B、C平面),203例胎儿获得肺动脉分支三维平面、198例获得主动脉弓和动脉导管弓三维平面。发现心脏畸形12例,其中大动脉转位2例;单心房单心室合并永存动脉干1例;房室通道畸形合并大动脉转位2例;室间隔缺损6例;法乐式四联症1例。结论 STIC技术可以通过对容积数据库的脱机分析,降低超声医师筛查胎儿心脏的操作难度,得到二维超声较难获得的胎儿大血管筛查切面,从而使胎儿先天性大血管病变的检出率得到提高。

Abstract:OBJECTIVE To evaluate the application of spatiotemporal image correlation(STIC)in fetal great vessel imaging.METHODS 245 pregnant women with 20-38 weeks fetuses were examined for heart of fetuses by the two-dimension ultrasound(PHILIPS IU-22 Color Doppler ultrasonography),then the spatiotemporal image correlation and acquired volume data was analyzed.RESULTS A11 the 245 fetuses were wel1 displayed by the three-dimensional graphic images(ABC flat)and the STIC volume data were gained(A,B and C surface).203 fetuses gained the 3 D graphic of pulmonary artery branch,198 gained the 3 D graphic of aortic arch and artery duct arch.The heart malformation of the 12 fetuses were found,2 cases showed TGA,1 case had single atrium single ventricle combined truncus arteriosus,2 cases had atrioventricular canal malformation combined TGA,6 cases had ventricular septal defect,and 1 case had tetralogy of fallot.CONCLUSION STIC can analyze the volume data outline and reduce operational difficulties of ultrasound screening for fetal heart,acquire the fetal great vessel imaging which could not be gained by two-dimension ultrasound,and improve detection rate of the fetal congenital great vessel disease.
