小切口非超声乳化治疗高度近视眼白内障的临床研究

目的:观察并探讨小切口非超声乳化白内障摘除联合后房型人工晶体植入术治疗高度近视白内障患者的效果和体会。方法:对61例(78只眼)高度近视白内障患者实施小切口非超声乳化白内障摘除联合后房型人工晶体植入术,术前记录眼轴长度,轴长≤29mm的51眼为A组,>29mm的27眼(34.6%)为B组,术后记录视力,观察手术并发症,术后眼部情况。术后随访时间3~6个月。结果:术后视力(包括纠正视力)较术前均有不同程度提高,术后1个月视力0.3以上者达73.1%,其A组42眼(82.4%),B组15眼(55.6%),2组比较差异有统计学意义(p<0.05)。结论:小切口非超声乳化白内障摘除联合后房型人工晶体植入是治疗高度近视白内障患者的一种既安全又经济的手术方式,尤其适合在基层开展。术后视力恢复情况与眼轴长度有关。

Clinical Effects of Small Incision Non-phacoemulcification Cataract Extraction with IOL Implantation in Cataract Patients with High Myopia

Objective:To evaluate the clinical effect and analyze the experience of small incision non-phacoemulcification cataract extraction with IOL implantation for cataract patients with high myopia.Methods: 61 cases (78 eyes) of cataract-complicated high myopia were treated with small non-phacoemulification cataract extraction with IOL implantation. Preoperative axial length, postoperative visual acuity and refractive error were observed and recorded. There were 51 eyes in axial length group ≤29mm and 27 eyes in axial length group >29mm.The follow-up time was 3 to 6 months.Results: Among the 57 eyes (73.1% ) which had a uncorrected or corrected visual acuity better than 0.3, 42 eyes (82.4%) were in the axial length group ≤29mm and 15 eyes (55.6%) were in axial length group >29mm. (p<0.05, the difference was satstically significant) .Conclusions: Small incision non-phacoemulcification cataract extraction with IOL implantation might be suitable for cataract-complicated high mypoia patients, especially in rural areas. All patients had a better visual acuity after the operation. The recovery for eyes is less satisfactory in eyes over 29mm axial lengths.
