电视胸腔镜治疗老年自发性气胸

目的：探讨电视胸腔镜治疗老年自发性气胸的临床价值。方法：电视胸腔镜监视下手术治疗60岁以上自发性气胸21例（VATS组），与单纯胸腔闭式引流治疗58例（胸引组）进行比较。结果：VATS组手术均获成功，无住院死亡。术后胸引时间、住院天数与胸引组比较，显著缩短，死亡率及复发率显著降低，8例呼吸衰竭的患者术后均得到恢复。结论：电视胸腔镜治疗老年自发性气胸仍为安全可靠的手术，并可降低死亡率，减少复发。

Video assisted thoracoscopic managoment of spontaneous pneumothorax in patients over 60 years of age

Objective:To evaluate the Video Assisted Thoracoscopic surgery (VATS) in the treatment of older patients with spontaneous pneumothorax.Methods:VATS was performed in 21 patients with the age over 60 and just treated by thoracic tube drainage in 58 patients as control. Results: All 21 cases of older patients with VATS treatment of spontaneous pneumothorax are successful without any mortailty.Postoperative drainage period,and hospital stay after surgery of the VATS group were 7.3 days and 14.6 days, which were significantly shorter, and the rate of replace and mortality are significantly lower than the control group. 8 old patients with respiratory failure had recovered gradually after VATS operation. Conclusion:VATS is a safe and effective treatment for old patients with spontaneous pneumothorax. The old patients with spontaneous pneumothorax should be treated earlier in order to reduce the mortality and recurrent rate.
