上海市闸北区糖尿病高危人群筛查情况分析

目的了解上海市闸北区糖尿病高危人群现状,对相关危险因素进行深入研究。方法采用多阶段分层整群随机抽样方法,依据危险因素记分法,明确糖尿病高危人群的分布情况,并对该人群的年龄、BMI、腰臀比、血压、家族史、空腹血糖和心血管疾病等情况,以及中心性肥胖、高血压和高血脂人群中糖尿病及高危人群的情况进行分析。结果该项目筛查出的糖尿病高危人群共有6043人,该人群中超重或肥胖、高血压和高血脂等比较普遍。腰臀比异常组与正常组之间空腹血糖构成情况无显著性差异,高血脂人群中糖尿病患者及糖调节受损检出率显著高于正常组,高血压组中糖尿病患者及糖调节受损检出率却显著低于正常组。结论通过对35岁以上人群初筛确定的高危人群血糖水平均值接近于糖调节受损水平。因此,有必要对高危人群进行早期健康管理,以降低糖尿病发病率,减少并发症发生率。

Study on Diabetes High Risk Adolescents in Zhabei District of Shanghai

OBJECTIVE To investigate the prevalence of diabetes high risk adolescents in Zhabei District of Shanghai and make an in-depth analysis on the risk factors.METHODS A cross-sectional study was conducted in Zhabei District,Shanghai and a representative sample was randomly selected using stratified cluster sampling.The risk factors scoring method proved by Shanghai CDC in 2002 was used.Besides,the risk factors such as age,BMI,WHR,blood pressure,family disease history,FPG,CVD and relationship between abdominal obesity or high blood pressure or hyperlipidemia and diabetes were analyzed.RESULTS There were 6 043 newly-found diabetes high risk adolescents through this health-screening program.Most of people in the high risk group were diagnosed as overweight/obesity,high blood pressure,high TC/TG.Risk analysis found that there were no significant differences of FPG distribution found between the high and normal WHR groups.However,FPG level in the high TC/TG group was significantly higher than the normal group while it was lower in the high BP group than the normal group.CONCLUSIONS The FPG level of diabetes high risk adolescents over 35 years old was close to the IFG standard.Thus,intervention measures such as community health management should be focused on the high-risk group in order to lower diabetes and relevant complications incidences.
