摘    要:目的:探讨临床应用组织追踪法定量分析新生儿左室心肌运动位移特征的价值。方法:应用组织追踪技术对22例足月儿和22例早产儿的室壁运动位移进行观察,将心尖部四腔心、二腔心测得的4个室壁3个节段之间的位移距离进行比较,对足月儿与早产儿的室壁位移距离进行比较。结果:足月儿、早产儿同一室壁收缩期位移距离由基底部向心尖部逐渐缩小。基底部、中部及心尖部 3 个不同水平的节段位移距离比较,差异有非常显著性意义(P<0.01)。足月儿4个室壁的基底部位移距离,与早产儿比较,差异有显著性意义(P<0.05),而中部、心尖部位移距离,足月儿与早产儿比较差异无显著性(P>0.05)。结论:组织追踪法是定量评价新生儿心肌运动的新方法。足月儿左室基底部的纵向位移距离大于早产儿。

Abstract:Objective: To quantitatively analyze the displacement of segmental movements of left ventricule with tissue tracking in neonates. Methods: The views was obtained at the apical four-chambers and two-chamber views, the displacement of segmental movements at the basal, middle and apical levels were measured and compared in term and preterm infants by tissue tracking. Results: The segmental displacement in the same ventricular wall became smaller progressively from basal to apical level in both of term and preterm infants. There were significant differences in the displacement of segmental movements among the basal, middle and apical levels (P<0.01). There were significant differences in the displacement at the basal level (P<0.05) and were no significant differences at the middle and apical level (P>0.05) between term and preterm infants. Conclusion: Tissue tracking is a new method which can noninvasively and quantitatively assess myocardial movements in neonates. The longitudinal displacement of the term infant’s left ventricular movements at the basal level is higher than that of preterm infants.
