Rt-PA和尿激酶溶栓治疗急性脑梗死的临床效果比较

目的:探讨Rt-PA和尿激酶溶栓治疗急性脑梗死的临床效果.方法:选择我院2009年3月-2011年4月急性脑梗死患者100例,随机分为两组,观察组50例,采用Rt-PA溶栓治疗,对照组50例,采用尿激酶溶栓治疗.观察两组溶栓效果和溶栓相关不良反应发生情况.结果:观察组血管再通率与对照组比较,差异无统计学意义(P＞0.05)；观察组总有效率为94.0％,对照组总有效率为92.0％,观察组治疗后总有效率与对照组比较,差异无统计学意义(P＞0.05)；两组溶栓后继发性出血发生率比较,差异有统计学意义(P＜0.05)；观察组溶栓再通患者中脑水肿发生率与对照组比较,差异无统计学意义(P＞0.05).结论:Rt-PA溶栓治疗急性脑梗死临床效果显著,溶栓后继发性脑出血发生率低,优于尿激酶溶栓治疗.

Clinical Effect Study on Intra-arterial Thrombolysis with Rt-PA and Urokinase for Acute Cerebral Infarction

Objective: To investigate the clinical effect of intra-arterial thrombolysis with urokinase and recombinant tissue plasminogen activator(rt-PA)for the treatment of acute cerebral infarction.Methods:Totally 100 cases of acute cerebral infarction hospitalized in Neurological Department of our Hopital from March 2009 to April 2011,which were randomized into experimental group and control group.rt-PA and urokinase were used respectively in experimental group and control group,and their clinical efficacy and adverse reactions were evaluated.Results:There were no significant differences(P>0.05)in revascularization rate between the experimental group(94.0%) and control group(92.0%).There were no significant differences(P>0.05)in efficacy rate between control group and post-treatment experimental group.There were significent differences(P<0.05)between two groups after intra-arterial thrombolysis.There were no significant difference(P>0.05)in cerebral edema rate between patients of thrombolysis recanalization in control group and experimental group.Conclusion:Rt-PA for acute cerebral infarction is better that with urokinase,expecially in Secondary cerebral hemorrhage rate.
