卵巢上皮性肿瘤的声像图特点与其肿瘤标志物定量的相关性研究

目的探讨卵巢上皮性肿瘤声像图特点与血清肿瘤标志物的相关性。方法对136例卵巢上皮性肿瘤患者的经阴道超声的二维声像图、血流信号强度和肿瘤内径3个指标分级，分析其与肿瘤标志物血清癌胚抗原（CA125）、骨桥蛋白（OPN）、恶性肿瘤特异性生长因子（TSGF）、溶血磷脂酸（LPA）的相关性。结果四项血清肿瘤标志物水平随超声信息分级的升高而增加，呈正相关（P〈0．01）。二维声像图与CA125、LPA呈高度相关（rs〉0．8），与OPN和TSGF呈中度相关（L〉0．6）；血流信号强度与LPA呈高度相关（rs〉0．8），与CA125、OPN、TSGF呈中度相关（rs〉0．4）；肿瘤内径与LPA呈高度相关（rs〉0．8），与CA125呈中度相关（rs〉0．6），与OPN和TSGF呈低度相关（rs〈0．4）。结论超声检查结合LPA、CA125的检测可能较结合OPN、TSG的检测，对卵巢上皮性肿瘤严重程度的判定更具特异性。

The correlation study of transvaginal ultrasound characteristics and serum tumor markers in epithelial ovarian carcinoma

Objective To study the relationship between transvaginal ultrasound information and serum tumor markers of epithelial ovarian carcinoma. Methods The data of transvaginal ultrasound information and serum tumor markers in 136 patients with epithelial ovarian carcinoma were retrospectively analyzed. Ultrasound information was divided into 3 indexes, including sonogram, bloodstream signal- gram and tumor inner diameter, then the relationship between indexes and serum tumor markers was analyzed. Results There was positive correlation between transvaginal ultrasound information and serum tumor markers （ P 〈 0.01 ） ; In sonograms, the correlation between indexes of CA125 and LPA was high （ rs 〉 0.8 ）, OPN and TSGF was moderate （ rs〉 O. 6）; In bloodstream signal - gram, the correlation of index of LPA was high（ rs 〉 0.8）, CA125, OPN and TSGF were moderate（ rs 〉 0.4） ; In tumor inner-diameter, the correlation of index of LPA was high（ rs〉 0.8）, CA125 was moderate（ rs 〉 0.6）, OPN and TSGF was lower（ rs〈 0.4）. Conclusion Transvaginal ultrasonography combined with CA125 and LPA is more specific than combined with OPN and TSGF in diagnosing epithelial ovarian carcinoma.
