胃癌及癌前病变中Bcl-2、Bax表达及其与Hp感染的关系

目的探讨胃癌及癌前病变胃上皮细胞的凋亡及幽门螺杆菌（Hp）感染致癌的可能机制。方法应用快速尿素酶试验与W—S银染色法将各种病变分为Hp感染阳性组和阴性组，应用免疫组化sP法对各种病变凋亡相关基因Bcl-2、Bax的表达进行检测。结果Bcl-2基因蛋白的表达从浅表性胃炎、肠化生到不典型增生逐渐增加，胃癌时表达率最高，而Bcl-2表达在Hp阳性及Hp阴性组无明显差异。Bax基因蛋白表达随病变进展逐渐降低，且在浅表胃炎、肠化生组Hp阳性者Bax表达明显高于Hp阴性者（P〈0．05）。结论Hp感染可通过促进Bax基因蛋白的过度表达而引起细胞凋亡增加和胃炎发生，且这种诱导凋亡的作用主要发生在胃黏膜病变的早期阶段。Bcl-2基因的异常表达对胃癌及癌前病变的形成发挥一定作用。

Bax, Bcl-2 protein expressions and their association with helicobactor pylori infection

Objective] To observe apoptosis of gastric epithelial cells in gastric carcinoma and precancerous lesion and further elucidate the possible effect of Hp infecton on gastric careifioma pathogenesis. Methods] The expressions of Bcl-2 and Bax were examined by immunohistochemical SP technique in gastric epithelial cells. Resuits] Of all patients,the expression of Bcl-2 was increased from CSG, IM to Dys, and reached a peak in GC, but there was no difference between Hp positive group and negative group. The expression of Bax was decreased from CSG to GC. In Hp positive CSG and IM groups the expression of Bax was higher than Hp negative groups. Conclusion] Hp infection may induce apoptosis and gastritis by the excessive expression of Bax, which mainly occur in the initiating stages in gastric musocal lesions. Alberrant Bel-2 expression plays certain role in gastric cancer and precancerous lesion formations.
