30岁及以下女性乳腺癌的临床特点和预后

探讨３０岁及以下女性乳腺癌的临床特点和预后。方法回顾分析１９５６～１９９３年手术治疗３０岁及以下单侧女性乳腺癌９５例。结果腋窝和内乳淋巴结转移率分别为５９．１％和３１．２％。局部复发率和远处转移率分别为３．２％和２７．４％，局部复发伴远处转移者占４．２％。伴妊娠及哺乳者２１例（２２．１％）。腋淋巴结阴性的妊娠哺乳期和非妊娠哺乳期乳腺癌患者术后的５年生存率分别为５７．１４％和８１．４０％，而腋淋巴结阳性者则分别为３８．１０％和７３．７０％。结论对３０岁以下乳腺癌患者应尽可能清除内乳淋巴结；妊娠哺乳期乳腺癌患者无论腋窝淋巴结有无转移，预后均较差；对腋窝淋巴结阴性者也应强调综合治疗。

CLINICAL FEATURES AND PROGNOSIS OF BREAST CANCER IN WOMEN AGED 30 AND UNDER

PURPOSE To understand the clinical features and prognosis of breast cancer in women aged 30 and under METHODS Data from 95 female patients aged 30 and under who received operation for breast cancer in our hospital from 1956 to 1993 were analysed. RESULTS The positive rates of axillary and internal mammary lymph node metastases were 59.1% and 31.2% respectively.Local recurrence and distant metastases were 3.2% and 27.4% respectively, 4 2% of the patients had local recurrence plus distant metastases.21 of the 95 patients were at the time of diagnosis.The 5 year survival rates of axillary node negative patients were 57.14% for the pregnancy associated group at the time of diagnosis, and 81.4% for those who were not of this group.The 5 year survival rates for axillary node positive patients were 38.1% for the same first category and 73.7% for the second. CONCLUSIONS Internal mammary lymph nodes should be dissected in patients aged 30 and under.The concurrent pregnancy or breastfeeding affects adversely the survival of patients irrespective of axillary lymph node status.Combined therapy should be emphasized in axillary node negative patients as well.
