囊状肺动静脉畸形的影像学诊断及栓塞治疗

摘    要:目的 分析肺动静脉畸形(PAVMs)的发病原因、临床表现,研究各种影像学方法在囊状PAVMs诊断中的应用,评价经导管栓塞治疗(TCE)囊状PAVMs的疗效。方法 回顾性分析17例行影像学检查和栓塞治疗的患者资料。结果 17例患者中,9例行超声心动图声学造影均提示PAVMs,13例具有典型的X线平片表现,全部患者肺动脉造影均清楚地显示病变的部位、大小、形态。17例患者TCE治疗全部成功,术后即刻平均股动脉血氧饱和度和耳血氧分压较术前显著升高。结论 肺动脉造影是诊断PAVMs的金标准;TCE安全、有效,是治疗囊状PAVMs的首选方法。

Abstract:Objective To analyze the etiology and clinical manifestations of pulmonary arteriovenous malformations(PAVM,), to study the value of different imaging diagnostic methods for saccular PAVMs, to evaluate the efficacy of transcather embolization (TCE) for saccular PAVMs. Methods The data of 17 patients with saccular PAVMs who underwent imaging diagnostic examinations and TCE were analyzed respectively. Results 9 cases underwent Contrast echocardiography were proven PAVMs. Plain chest radiograph showed classic features in 13 of 17 patients with saccular PAVMs. Pulmonary angiography defined the size, location and anatomical details of PAVMs. TCE was successfully performed in all patients. There was a statistically significant rise in mean oxygen saturation and oxygen tension immediately after TCE. Conclusion Pulmonary angiography is a the gold standard in the diagnosis of PAVMs. TCE is a safe and effective procedure for treatment PAVMs.
