高血压患者颈动脉超声与脑循环动力学的关系

摘    要:目的:探讨高血压病颈动脉粥样硬化与脑循环动力学之间的关系。方法:检测分析586例高血压病患者的颈动脉内膜中层厚度(IMT),斑块发生率及脑循环动力学(CVA)并与200例健康成人对照。结果:随颈动脉IMT增厚和斑块分级增加,患者脑平均血流速度(Vmean),脑平均血流量(Qmean)明显降低,而脑血管床特性阻抗(Zc),管壁弹性波波速(Wv)及外周阻力(R)明显升高,颈动脉IMT与CVA密切相关。结论:颈动脉粥样硬化是造成脑供血障碍和脑循环动力学异常的重要因素,颈动脉超声检测对于防治高血压合并脑血管病具有重要意义。

Abstract:Objective:To investigate the relation between carotid atherosclerosis and cerebrovascular hemodynamics analysis (CVA)in patients with essential hypertension.Methods:Carotid artery intima media thickness(IMT),Carotid artery plaque and CVA were measured in 586 patients with essential hypertension compared with 200 controls.Results:Carotid artery IMT and plaque occurrent rates were higher in essential hypertersion than those of controls,Vmean and Qmean were lower and Zc?Wv and R were higher in patients than those of controls.There were significant differences of carotid artery IMT and CVA in groups of essential hypertension.Conclusions:The carotid artery IMT was closely correlated with CVA.The carotid atherosclerosis could cause the abnormal phenomena of cerebrovascular hemodynamics and circulation.It is important to apply carotid artery ultrasonography in the treatment and prevention of cerebrovascular disease.
