重组组织型纤溶酶原激活物早期静脉溶栓治疗急性脑梗死的临床研究

目的探讨重组组织型纤溶酶原激活剂早期静溶栓治疗急性脑梗死的疗效及安全性。方法回顾性分析笔者所在医院80例急性脑梗死患者的临床资料,发病时间<6h的患者给予rt-PA50mg静脉溶栓治疗,溶栓前及溶栓后30min、24h、14d及3个月分别采用美国国立卫生院卒中量表(NIHSS)评分,溶栓后3个月给予修订的Rankin评分和日常生活能力Barthal指数评分,评价其疗效及安全性。结果溶栓后各时间点NIHSS评分均有明显改善,组间比较无显著性差异(均P>0.05);≤6h治疗其1h、3h、24h再通率效果显著,明显优于另两组(均P<0.05);≤6h治疗再出血的发生情况明显低于另两组(均P<0.05)。结论 ACI发病6h内给予rt-PA静脉溶栓治疗相对安全有效。

Clinical Study of Early Intravenous Thrombolysis Treatment of Recombinant Tissue-type Plasminogen Activator in Patients with Acute Cerebral Infarction

Objective To evaluate the efficacy and safety of recombinant tissue plasminogen activator thrombolytic therapy for acute cerebral infarction early static.Methods Analysed 80 cases clinical data in patients with acute cerebral infarction retrospectively.Incidence time of the ACI patients<6h were treated with rt-PA(50mg) of intravenous thrombolytic therapy thrombolysis,before and after thrombolysis 30min,24 h,14d and 3 months were used by the U.S.National Institutes of Health Stroke Scale score,and 3 months after thrombolysis,given the revised Rankin score and activities of daily living Barthal Index score,to evaluate the efficacy and safety.Results NIHSS scores were significantly improved at different time points after thrombolysis,no significant difference between groups(all P>0.05);≤6h treatment of the 1h,3h,24h a significant effect of recanalization rate was significantly better than the other two groups,(all P<0.05);≤6h treatment the incidence of rebleeding was significantly lower than the other groups(all P<0.05).Conclusion Give rt-PA thrombolytic therapy to patients of ACI within 6h is relatively safe and effective.
