急性心肌梗死患者IABP后下呼吸道感染的非条件Logistic回归分析

目的分析急性心肌梗死患者实施主动脉内球囊反搏术(IABP)后发生下呼吸道感染的危险因素。方法选取2012年5月至2013年4月天津市南开医院收治的156例急性心肌梗死行IABP治疗患者的临床资料,根据是否发生术后下呼吸道感染分为感染组(46例)和非感染组(110例),对两组患者的临床资料进行单因素和多因素分析,总结急性心肌梗死行IABP治疗后患者发生术后下呼吸道感染的危险因素。结果行IABP治疗的156例患者中有46例发生术后下呼吸道感染,感染率为29.49%;术后发生下呼吸道感染的独立危险因素为:高龄、合并糖尿病、白细胞计数高、深静脉置管、有创通气、使用多种抗菌药物。结论急性心肌梗死患者在实施IABP治疗时应充分考虑患者的年龄,综合评估患者采取该手术进行治疗时发生术后下呼吸道感染的风险,并采取相应的医疗措施以降低感染风险。

The Non-conditional Logistic Regression Analysis on the Lower Respiratory Tract Infections of Acute Myocardial Infarction Patient Treated by IABP

Objective To analyze the risk factors of the lower respiratory tract infections on AMI patient treated by intra-aortic balloon pump（IABP）. Methods Retrospective analysis of 156 AMI patients treated by IABP in Nankai Hospital from May 2012 to Apr. 2013 was done. The patients were divided into 2 groups based on the postoperative lower respiratory tract infections ： infection group （46 cases） and non-infection group（ 110 cases）, the clinical data of these patients were analyzed with the non-conditional logistic regres- sion, and the risk factors of lower respiratory tract infections among AMI patients treated by IABP were summarized. Results Total of 46 patients suffered lower respiratory tract infection, the infection rate was 29.49%, the independent risk factors of postoperative lower respiratory tract infection were advanced age, combination with diabetes, high leukocyte count, deep venous catheter, invasive mechanical ventilation, use of multiple antibiotics. Conclusion The age of the AMI patients to be treated by IABP should be taken into comprehensive consideration, and the risk of lower respiratory tract infection should be aasessed, and corre- sponding medical measures should be taken to reduce the risk of infection.
