腹腔镜手术对良性卵巢肿瘤患者内皮素、IL-6及C-反应蛋白的影响

目的]研究腹腔镜手术对卵巢良性肿瘤患者机体应急反应的影响。方法]2007年9月～2008年9月60例卵巢良性肿瘤患者随机分为腹腔镜组30例和开腹手术组30例。比较两组术前和术后1、3、5d内皮素（ET）、白细胞介素（IL-6）、C-反应蛋白（CRP）的变化。结果]腹腔镜组术后第1dET显著下降（P〈0．05），术后第3d恢复至术前水平：而开腹组手术前后ET无显著性变化。两组患者术后IL-6、CRP水平均增高（P〈0．01），且开腹组显著高于腹腔镜组（P〈0．01），术后第5d腹腔镜组IL-6恢复至术前水平；术后第5d两组CRP水平仍显著高于术前水平（P〈0．05）．且开腹组显著高于腹腔镜组（P〈0．05）。结论]腹腔镜手术对卵巢良性肿瘤患者相比开腹手术应急反应轻．强度小．持续时间短。

Impact of Laparoscopic Surgery on Endothelin, IL-6 and C-re-active Protein in Patients with Benign Ovarian Tumor

Purpose] To investigate the impact of laparoseopie surgery on body emergency response in patients with benign ovarian tumor. Methods] From Sept. 2007 to Sept. 2008, 60 cases with benign ovarian tumor were randomly divided into laparoscopic group （n=30） and open surgery group （n=30）. The changes of endothelin （ET）, interleukin-6（IL-6） and C-reactive protein （CRP） at preoperation and 1,3 and 5 day postoperation in the groups were analyzed. Results] ET in laparoscopic group decreased significantly at the first postoperative day（P〈0.05）, and return to preoperative level at 3 day postoperation; but ET level was no significant changes before and after operation in open surgery group. The postoperative IL-6 and CRP levels in both groups were increased significantly（P〈0.01）, and they were higher in open surgery group than those in laparoscopic group（P〈0.01 ）; the IL-6 level in laparoscopic group was return to preoperative level at the 5th postoperative day. The CRP level in both groups on the 5th postoperative day was still higher than that before operation （P〈0.05）, and it was higher in open surgery group than that in laparoscopic group（P〈0.05）. Conclusions] Compared with open surgery, laparoscopic surgery for patients with benign ovarian tumor was less body emergency response, less intensity with short duration.
