MRI和CT在食管癌TN分期中的价值比较

目的:比较MRI和CT检查对食管癌术前TN分期的价值。方法:按纳入标准收集我院2012年10月至2014年11月经手术病理确诊的食管癌患者60例,同一患者均在术前行MRI和CT检查,分析所有患者术前MRI和CT两组检查对食管癌TN分期的价值,并与术后病理结果进行对照。结果:MRI和CT诊断T1-2期的准确率分别为62.07%和31.03%,两者差异有统计学意义(P<0.05);MRI和CT诊断T3-4期的准确率没有统计学意义(P>0.05),均为83.87%;MRI和CT在纵膈N0和N1的准确率均没有统计学意义(P>0.05),分别为75.76%和81.81%,70.37%和77.78%,MRI和CT在上腹部N0和N1的准确率均没有统计学意义(P>0.05),分别为84.62%和87.18%,76.19%和80.95%。结论:MRI评估T1-2期的食管癌分期明显优于CT,对T3-4期和N分期MRI和CT检查价值相当,两者均可为术前判断食管癌TN分期提供可靠依据。

Compare the diagnostic value of TN staging of esophageal cancer by MRI and CT

Objective:To compare clinical implication of MRI and CT in preoperative esophageal cancer TN staging. Methods:According to inclusion criteria,sixty patients with esophageal cancer were underwent MRI and CT examination before the operation from October 2012 to November 2014. The clinical implication of MRI and CT was evaluated in preoperative esophageal cancer TN staging, which was compared with the findings of the operation and pathological examination for resected specimens. Results:There were signifi-cant differences between MRI and CT in evaluating T1-2 stage(P <0. 05),which the accuracies of diagnosis were 62. 07% and 31. 03%,respectively. The accuracies of T3-4 stage were 83. 87% both MRI and CT,which were no significant differences(P>0. 05). For chest regional lymph nodes stage,the accuracies of diagnosis were no significant differences in N0 and N1 between MRI and CT (P>0. 05),and they were 75. 76% and 81. 81%,70. 37% and 77. 78%,respectively. For abdomen regional lymph nodes stage,there were also no significant differences in N0 and N1 between MRI and CT(P>0. 05),and they were 84. 62% and 87. 18%,76. 19% and 80. 95%,respectively. Conclusion:The clinical implication of MRI is superior to CT in evaluating T1-2 stage of esophageal cancer. There are no differences on evaluating T3-4 stage and regional lymph nodes stage of esophageal cancer by MRI and CT,which both can provide reliable basis for preoperative esophageal cancer TN staging.
