压力调节容量控制通气治疗呼吸窘迫综合征新生儿时改良的护理措施

目的:观察压力调节容量控制通气模式(pressure regulated volume control ventilation,PRVC)治疗呼吸窘迫综合征(respiratory distress syndrome,RDS)新生儿时改良护理措施的效果。方法:选择2012年1月至2013年12月在淮安市妇幼保健院新生儿医学中心住院的RDS新生儿作为研究组(n=80,采用PRVC模式通气和实施改良的护理措施);选择2011年1月至2011年12月在该院住院治疗的RDS新生儿作为对照组(n=93,采用PRVC模式通气和实施常规护理措施)。比较两组在机械通气时间、住院时间、呼吸机相关肺炎(VAP)发生率间的差异。结果:研究组患儿均痊愈出院,机械通气时间为(76.45±36.35)h,住院时间为(15.87±8.44)d,呼吸机相关肺炎发生率为15.0%,均低于对照组,差异有统计学意义(P均<0.05)。结论:PRVC治疗RDS新生儿时,改良的护理措施能缩短患儿机械通气时间和住院时间,降低VAP发生率,提高RDS新生儿的救治质量。

The improved care measures in neonates with respiratory distress syn-drome ventilated by the pressure regulated volume control ventilation

Objective:To discuss the efficacy of the improved care measures in the treatment of neonatal respiratory distress syndrome( RDS) by the pressure regulated volume control ventilation( PRVC) mode. Methods:The 80 cases of neonates with RDS who were treated by PRVC mode and the improved care measures in Huaian Maternity and Child Healthcare Hospital from January 2012 to December 2013(study group)and the 93 cases of neonates with RDS who were treated by PRVC mode and the routine care measures in the same hospital from January 2011 to December 2011(control group)were included. The duration of mechanical ventilation,the hospi-talization time,the incidence rate of ventilator-associated pneumonia( VAP) were observed in the two groups. Results:The 80 cases of neonates in the study group were recovered. The average duration of mechanical ventilation,the average hospitalization time and the in-cidence rate of VAP in the study group were all lower than those in the control group(P<0. 05). Conclusion:The improved care meas-ures of PRVC in neonates can reduce the duration of mechanical ventilation the hospitalization time and the incidence of VAP and im-prove the quality of treatment of RDS in neonates.
