温针群刺法治疗肱骨外科颈骨折术后肩关节活动功能障碍

目的观察温针群刺联合康复训练治疗肱骨外科颈骨折术后肩关节活动功能障碍的临床疗效。方法 2007年8月2009年11月,将68例肱骨外科颈骨折术后患者随机分为温针群刺加运动组（37例）,运动疗法组（31例）。温针群刺加运动组取患肩部僵硬软组织进行群刺,并行红外线照射;配穴取曲尺、合谷、阳陵泉,予针刺治疗,取针后活动肩关节。运动疗法组先被动活动肩关节后再行主动运动训练。治疗50d后统计疗效。结果与治疗前比较,治疗结束后两组患者的VAS、肩关节功能、活动度、活动度评分,均有统计学意义（P〈0.05）。治疗后与运动组比较,温针群刺加运动组患者的肩关节疼痛、功能、活动度、活动度评分,均有统计学意义（P〈0.05）。结论两种治疗方法对骨折术后肩关节的功能恢复均具有较好的临床疗效。与运动康复治疗相比较,温针群刺加运动训练对骨折术后肩关节的功能恢复具有更显著的效果。

Effectiveness of Warm-Group-Needle and Rehabilitation in the Treatment of 68 Cases with Shoulder Dysfunction after Fracture of the Lateral Condylar Neck

Objective To observe the clinical effectiveness of warm-group-needle and rehabilitation in the treatment of shoulder dysfunction induced by fracture of the lateral condylar neck. Methods All 68 patients with shoulder dysfunction induced by fracture of the lateral condylar neck were divided randomly into a warm-group-needle and rehabilitation group （n=37） and a rehabilitation group （n=31） from August 2007 to November 2009. The warm-group-needle and rehabilitation group were acupunctured in the rigid part of the wicked shoulder, at the same time using infrared radiation, assistant acupoints were quchi, hegu, yanglingquan; after acupuncture, some shoulder rehabilitation exercise was done. The rehabilitation group did some passive shoulder rehabilitation exercise firstly and then some active shoulder rehabilitation exercise. The treatment was analyzed after 50 days. Results Compared with scores before treatment, there was significant difference in the VAS of the shoulder, function, ROM, and the score of ROM after treament （P〈0.01）. After treatment, compared with the single rehabilitation, the warm-group-needle and rehabilitation was found with significant difference in the VAS of the shoulder, function, ROM, and the score of ROM （P〈0.01）. Conclusion The two treatments can improve the function of the shoulder dysfunction induced by fracture of the lateral condylar neck. Compared with the single rehabilitation treatment, warm-group-needle and rehabilitation is more effective.
