纳洛酮对颅脑损伤患者脑摄氧率的影响

摘    要:目的 观察纳洛酮给药对颅脑损伤患者脑氧摄取率的影响。方法 选取2001年1月—2006年12月颅脑损伤入院患者86例,随机分成纳洛酮治疗组和对照组,两组患者性别、年龄及入院GCS评分均无明显差异(P>005)。两组均于伤后当天至第7天经颈静脉球部及股动脉同步抽取血样进行血气分析,计算出脑氧摄取率,进行统计学处理。结果 在治疗后1~7 d,纳洛酮治疗组与对照组脑氧摄取率呈明显下降趋势,治疗组轻型和中重型颅脑损伤患者的脑氧摄取率明显高于对照组,有统计学意义(P<005);两组死亡患者脑摄氧率比较,治疗组与对照组无差异(P>005)。结论 纳洛酮可提高颅脑损伤患者的脑氧摄取率,明显改善患者预后。

Effects of Naloxone on Cerebral Extraction Rate of Oxygen in Heavy Craniocerebral Injury Patients

Abstract:Objective To observe the effects of naloxone on cerebral extraction rate of oxygen (CEO2) in craniocerebral injury patients. Methods Eighty-six patients with heavy craniocerebral injury were divided into naloxone treatment group and control group randomly. There were no significant differences between two groups in sexuali- ty, age and score of CJCS (P〈0.05). Blood hemospasia in bulbus venae jugularis and arteria cruralis were analysed from the first day to the seventh day, and CEO2 was calculated to make significance test. Results During the first day to the seventh day after treatment, CEO2 decreased in two groups, and the rate in naloxone treatment group was significantly higher than control group (P〈0. 05) ; the CEO2 in the dead of two groups had no significant difference (P〉 0. 05). Conclusion Naloxone treatment will rise CEO2 and improve the prognosis of craniocerebral injury patients.
