MT-3基因甲基化对食管鳞状细胞癌发病及预后的影响研究

摘    要:目的 探讨MT-3基因甲基化对食管鳞状细胞癌发病及预后的影响.方法 选取行食管癌切除手术,术后经病理证实为食管鳞状细胞癌且病理及随访资料完整的110例病例,分别对选取的肿瘤组织、正常组织和转移淋巴结标本蜡块进行常规连续切片、免疫组化染色、提取DNA并扩增DNA样本.应用重亚硫酸钠处理限制性内切图谱分析MT-3甲基化情况,统计分析其与食管鳞状细胞癌患者的分化程度、淋巴结转移数目及生存率的关系.结果 食管鳞状细胞癌组织中的MT-3表达水平高于正常的食管组织,并且在中分化和高分化的食管癌组织中MT-3呈现较明显的高表达,而低分化食管癌组织中MT-3的表达不明显.出现MT-3基因甲基化的患者淋巴结转移数量多于未出现MT-3基因甲基化的患者,并且出现MT-3基因甲基化的患者生存时间和2年生存率均较未出现甲基化的患者差,差异均有统计学意义(P﹤0.05).结论 MT-3基因的表达程度和甲基化程度与食管鳞状细胞癌的病情和预后评估密切相关,有望将其作为病情和预后评估的指标之一.

Abstract:Objective To investigate the effect of methylation of MT-3 gene on the pathogenesis and prognosis of esophageal squamous cell carcinoma (SCC). Method 110 cases of esophageal cancer that were administered with surgi-cal resection, with pathologically confirmed esophageal SCC and complete pathological and follow-up data, were includ-ed in the study, selected tumor tissues, normal tissues and lymph node metastases specimens were collected and processed with paraffin, routine serial sectioning and immunohistochemical staining, followed by DNA extraction and amplifica-tion. Sodium metabisulfite was used to treat restriction endonuclease map to analyze the methylation in MT-3, while statis-tical analysis was performed to evaluate the association between methylation and degree of differentiation, number of lymph node metastases, and survival rate. Result The expression of MT-3 gene in esophageal SCC tissues was signifi-cantly higher than that of normal esophageal tissues, and was high in MT-3 of moderately and well-differentiated esopha-geal carcinoma, while was low in the MT-3 of poorly differentiated cancer tissues. Patients with MT-3 gene methylation had more lymph node metastases, shorter survival time and lower 2-year survival rate than those without MT-3 methyla-tion (P<0.05). Conclusion The expression and methylation of MT-3 gene is closely related to the prognosis of esopha-geal SCC, and it can be used as one of the indicators for disease and prognosis evaluation.
