三叉神经痛、面肌痉挛的影像学表现与手术结果对照研究(附315例报告)

目的:分析总结三叉神经痛和面肌痉挛的影像学表现,经与手术结果对照,探讨三叉神经痛和面肌痉挛的主要致病原因。方法:回顾性分析2001年～2006年我院收治的315例三叉神经痛和面肌痉挛患者的有关资料。结果:术前CT平扫67例,15例显示椎基底动脉迂曲、增粗、偏移,占22.4%。248例均做桥小脑角区MRI,显示血管神经接触或可疑接触者188例,占75.8%。手术病例296例,术中证实有血管压迫或接触的271例,占91.6%;肿瘤压迫所致20例,占6.8%;有蛛网膜粘连和其它原因不明者5例,占1.7%,19例为保守治疗。结论:MRI诊断三叉神经痛和面肌痉挛的病因与手术结果对照最接近,则主要为周围血管压迫神经所致。

Comparative study between imaging finging and operation in trigeminal neuralgia and hemifacial spasm: a report of 315 cases

Objective:To investigate the primary causes of TN and HFS via studing the imaging funding of Trigeminal Neuralgia (TN) and Hemifacial Spasm (HFS) and comparing with the results of operations. Methods:315 cases of TN and HFS,which had been cured between 2001 to 2006, were repospectiney reviewed.Results:In 67 patients of CT, 15 cases were found the basiblar artery (22.4%). In 248 patients of MRI, 188 cases were found the responsible blood vessels, the rate was 75.8%. 296 cases received operation, 271 cases were found responsible blood vessels(91.6%), 20 cases were found cerebellopontive angle tumors(6.8%), 5 cases were identified as arachnoid adhesion or indefinite causes (1.7%). Another 19 cases were cured with drugs. Conclusion:The imaging diagnosis is as same as the results of operation. The primary causes of TN and HFS are the compression coming from the responsible blood vessel or tumor.
