成人腺病毒7型感染的临床特征分析

目的：分析2019年某部队一起腺病毒7型感染的临床特征，感染后WBC、PCT和淋巴细胞亚群的变化，为腺病毒感染的防控和临床诊疗提供参考。方法：对所有暴露的524名士兵展开病例调查，RT-PCR检测 ADV核酸，连续监测30天，对血常规、PCT和淋巴细胞亚群进行统计学分析。结果：累计确认73名HAdV-7感染者全部为男性，年龄19-33岁，发病率最高的为一连（23.8%）；临床症状以发热（100%），咽痛（68%）为主；感染组WBC升高，PCT明显升高，总CD3+T细胞和CD3+CD4+T细胞比例，CD4/CD8比值均降低，差异均有统计学意义（P<0.05）；感染第6天WBC达最高值，第3天PCT达最高值，第3，6，9天总CD3+T细胞，CD3+CD4+T细胞比例和CD4/CD8比值逐渐降低。结论：人腺病毒7型感染发病急，以发热、咽痛等症状为主，WBC、PCT、淋巴细胞亚群随着病情发展均会改变。本研究首次对腺病毒感染进行连续监测，为其防控和诊疗提供依据。

Clinical features analysis of adenovirus type 7 infection in adults

Objective: To analyze the clinical characteristics of adenovirus type 7 infection in an army in 2019, and the changes of WBC, PCT and lymphocyte subsets after infection, so as to provide reference for the prevention and control of adenovirus infection and clinical diagnosis and treatment. Methods: A case investigation was carried out on all the exposed 524 soldiers, the adv nucleic acid was detected by RT-PCR, and the clinical characteristics, blood routine, PCT and lymphocyte subpopulations were statistically analyzed for 30 days. Results: A total of 73 infected patients with HAdV-7 were male and 19-33 years old, with the highest incidence (23.8%). The main clinical symptoms were fever (100%) and pharyngeal pain (68%); in the infection group, the rise of WBC and PCT were significantly difference, and the proportion of total CD3+T cell and CD3+CD4+T cell and the ratio of CD4/CD8 was decreased and the difference was statistically significant (p <0.05); the highest value of WBC at day 6, the highest value of PCT in the third day, the total CD3+T cells in the 3rd, 6th and 9th day, the ratio of CD3+CD4+T cells and the CD4/CD8 ratio decreased. Conclusion: Human adenovirus type 7 infection is onset of acute, the main symptom is fever and sore throat. WBC, PCT and lymphocyte subsets change with the progression of the disease. In this study, adenovirus infection was monitored continuously for the first time, and it provides the basis for prevention and control, diagnosis and treatment.
