腹腔镜下输卵管切除与胚胎移植间隔时间对妊娠结局影响的研究

目的探讨输卵管积液患者腹腔镜下输卵管切除术与胚胎移植间隔时间对妊娠结局的影响。方法回顾性分析2017年1月至2019年3月在西北妇女儿童医院生殖中心因不孕症合并输卵管积液行腹腔镜下输卵管切除术后实施IVF-ET的128例患者,按手术与IVF-ET的间隔时间分为3组。组Ⅰ(60例):手术与胚胎移植间隔1个月;组Ⅱ(36例):手术与胚胎移植间隔2个月;组Ⅲ(32例):手术与胚胎移植间隔3个月或以上。主要观察指标为临床妊娠率和活产率,次要观察指标为HCG阳性率、流产率和早产率。比较观察指标在各组患者中的差异。结果3组患者临床妊娠率分别为61.67%、72.22%和68.75%,组间比较无显著性差异(P=0.541);活产率分别为51.67%、58.33%和68.75%,组间比较亦无显著性差异(P=0.286)。经多元回归分析后,3组间主要观察指标(临床妊娠率和活产率)和次要观察指标(HCG阳性率、流产率和早产率)比较仍无显著性差异(P>0.05)。结论不孕症合并输卵管积液患者腹腔镜下输卵管切除术与胚胎移植间隔时间并不影响妊娠结局。

Effect of interval between laparoscopic salpingectomy and embryo transfer on pregnancy outcome

Objective:To investigate the effects of the interval between laparoscopic salpingectomy and embryo transfer on pregnancy outcomes.Methods:The data of 128 infertile patients who were performed laparoscopic salpingectomy due to infertility and hydrosalpinx before IVF-ET from January 2017 to March 2019 were retrospectively analyzed.The patients were divided into three groups according to the interval time between operation and IVF-ET:groupⅠ(60 patients)with one month,groupⅡ(36 patients)with two months,and groupⅢ(32 patients)with three months or above.Main outcome measures were clinical pregnancy rate and live birth rate.The secondary outcomes measures were HCG positive rate,abortion rate and preterm delivery rate.The differences of observation indexes in each group were compared.Results:The clinical pregnancy rates of the three groups were 61.67%,72.22%and 68.75%respectively,there was no significant difference(P=0.541).The live birth rates were 51.67%,58.33%and 68.75%respectively,there was also no significant difference(P=0.286).The multiple regression analysis showed that there were no significant differences in clinical pregnancy rate,live birth rate,HCG positive rate,abortion rate and preterm delivery rate among the three groups(P>0.05).Conclusions:The interval between laparoscopic salpingectomy and embryo transfer does not affect pregnancy outcomes in infertility patients with hydrosalpinx.
