摘    要:【目的】探讨CT引导下肺内肿块穿刺活检术的临床应用价值。【方法】回顾分析本院2005年7月至2008年12月350例肺内肿块穿刺活检病例,采用美国COOK公司生产18G-20G切割式自动活栓枪穿刺活检。【结果】穿刺成功347例,不成功3例,取得明确病理诊断结果331例,确诊率94.6%,其中鳞癌183例、腺癌107例、未分化癌14例、转移癌6例,淋巴瘤2例、结核11例、炎性假瘤7例、脓肿1例,不定性16例。【结论】CT引导下经皮肺穿刺活检术定位准确、操作技术简单易行、损伤较小,对肺内肿块的穿刺成功率及准确率均较高,具有重要的临床应用价值。

Abstract:Objective]To explore the clinical value of pneumocentesis of lung phymatosis di'rected by CT. Methods]A total of 350 cases of pneumocentesis of lung phymatosis in our hospital from July 2005 to December 2008 were reviewed and analyzed. Cutting trough type autobiopsy gun (18G-20G) was used for the puncture biopsy. Results]There were 347 cases of successful puncture and 3 cases of unsuccessful puncture. There were 331 cases of definite diagnostic result, among which there were 183 cases of scale carcinoma, 107 cases of glandular cancer, 14 cases of undifferentiated carcinoma, 6 cases of metastatic carcinoma, 2 cases of lymph-gland tumor, 11 cases of tuberculosis, 7 cases of inflammatory pseudotumour and 1 case of phymatosis. The definite diagnosis rate reached 94.6 % and there were 16 cases of indefiniteness. Conclusion]Pneumocentesis directed by CT is simple and easy to handle, and has minimal invasion and accurate location. It is of great clinical value for successful and accurate puncture of lung phymatosis.
