腹腔镜电针Vater壶腹戳孔导管扩张术和Oddi括约肌切开术的初步探讨

目的 :探讨腹腔镜电针Vater壶腹戳孔导管扩张术和腹腔镜电针Oddi括约肌切开术治疗Oddi括约肌狭窄的手术方法。方法 :采用腹腔镜胆总管探查结合电针 ,套装胆道扩张导管和斑马导丝 ,为 13例胆管结石 ,胆管扩张合并Oddi括约肌狭窄患者行腹腔镜电针Vater壶腹戳孔导管扩张术。结果 :13例手术均获成功 ,无胆漏、出血、穿孔及胰腺炎。结论 :选择合适病例 ,腹腔镜胆总管探查术中采用腹腔镜电针Vater壶腹戳孔导管扩张术和腹腔镜电针Oddi括约肌切开术治疗Oddi括约肌狭窄有效、可行。

Discussion of laparoscopic ampulla of vater stabbing catheter dilation and laparoscopic oddi sphincterotomy

Objective:To study the methods of the operation of the handling of the stenosis of Oddi sphincter for laparoscopic ampulla of Vater stabbing catheter dilation and laparoscopic Oddi sphincterotomy.Methods:It was first adopted for pin head like electro knife to ampulla of Vater stabbed and catheter dilated until the papillary stenosis was released,then cut through Oddi sphincte by electro needle to track along the left side of catheter for a considerable distance until the papillary stenosis was expanded.Results:13 patients had obtained successful result that the dilatation and expansion of the stenosis of Oddi sphincter reached the expected purpose.No bile leakage,hemorrhage,perforation,peritonitis and pancreatitis occurred.Conclusions:It is safe and effective for patients who had completed successfully duct clearance and catheter dilation of the stenosis of Oddi sphincter with electro needle Oddi sphincterotomy by laparoscopic CBD exploration.
