儿童单纯性肥胖临床与血脂血糖代谢的改变

目的探讨儿童单纯性肥胖症对血脂、血糖、血压及肝脏的影响,为儿童单纯性肥胖症的防治工作提供依据。方法对62例单纯性肥胖儿童与55例正常同年龄儿童进行血压测量,应用生化分析仪检测血糖,血脂,包括三酰甘油（TG）、总胆固醇（TC）、低密度脂蛋白（LDL）、高密度脂蛋白（HDL）,应用超声诊断仪进行肝脏检查。结果 1.单纯性肥胖组与对照组儿童年龄与身高无显著差异,而体质量与体质量指数（BMI）、血压（BP）两组间差异均有非常显著意义（p〈0.001）。2.单纯性肥胖儿童血浆TC、TG、LDL、血糖的浓度明显高于对照组,差异均有显著性（p〈0.001,p〈0.05）,HDL浓度两组比较其差异无统计学意义（p〉0.05）。3.单纯性肥胖22例（35%）儿童肝脏B超显示存在脂肪肝。结论单纯性肥胖儿童存在血脂及血糖代谢紊乱,预防动脉粥样硬化性心血管疾病及糖尿病必须从儿童时期开始。

Study on Blood Lipid, Blood Glucose Metabolism and Fatty Liver in Children with simple Obesity

Objective To investigate the influence of simple obesity on serum lipids,blood glucose,blood pressure and liver of children and to provide evidence for the prevention and control of this disease.Methods 62 children with simple obesity and 55 normal children with the same age underwent measurement of blood pressure.Biochemical analyzer was applied to detect blood glucose and blood lipid,including triglycerides（TG）,total cholesterol（TC）,low density lipoprotein（LDL）and high density lipoprotein（HDL）;and ultrasound diagnostic equipment was used for liver examination.Results 1.There was no significant differences in age and height between children with simple obesity and control group,while body weight,body mass index（BMI）and blood pressure（BP）were significantly different between two groups（P〈0.001）.2.Plasma TC,TG,LDL and blood glucose concentration were significantly higher in children with simple obesity than controls（P〈0.001,P〈0.05）,there was no statistically significant difference in HDL levels between two groups（P〉0.05）.3.22 children with simple obesity（35%）showed fatty liver through liver ultrasonic image.Conclusion There is metabolic disturbance in blood lipids and blood glucose in children with simple obesity,suggesting the prevention of atherosclerotic cardiovascular disease and diabetes must begin from childhood.
