| 摘 要： | 目的探讨胃肿瘤性穿孔的外科治疗方法.方法对1990年4月至2000年4月间本院收治的25例肿瘤性胃穿孔患者的临床资料进行回顾性分析.结果穿孔修补(或加胃肠吻合手术)11例,姑息性切除8例,根治性切除6例.术后发生并发症4例,死亡2例.穿孔修补、姑息性切除、根治性切除术后1,3年生存分别为1和0例、6和1例、5和3例.三种手术方式的中位生存期分别为5,16,28个月.结论预防胃穿孔的发生和胃穿孔后积极的术中处理与术后的综合治疗措施,可望延长患者的生存时间和提高生存质量.

| Abstract: | Objective To probe the surgical treatment for perforation of gastric tumour.Methods The clinical data of 25 patients with perforation of gastric tumour which underwent different surgical procedures from April 1990 to April 2000 were analysed retrospectively.Results Eleven patients underwent repair of perforation,8 received partial gastrectomy and 6 radical gastrectomy.Postoperative complication occurred in 4/25,and mortality was 2/25.1-and 3-year survival rates were 1 and 0,6 and 8,5 and 3 in repair,partial gastrectomy and radical gastrectomy groups,respectively.The median survival time was 5,16,28 months,respectively.Conclusion Avoiding perforation and active treatment after perforation can prolong the survival time and improve the quality of life of the patients with perforation of gastric carcinoma. |
