妊娠晚期合并重型肝炎55例临床分析

目的 探讨妊娠晚期合并重型肝炎的病原学、分型、分期、分娩方式与产妇存活率的关系。方法 对55例妊娠晚期合并重型肝炎的临床资料进行回顾性分析。结果 妊娠晚期合并重型肝炎以乙型肝炎发病率最高，占85．45％，慢性重型肝炎占83．6％，急性、亚急性存活率明显高于慢性（P〈0．01），剖宫产存活率明显高于阴道分娩（P〈0．01），早、中、晚接受治疗成活率分别为90％、63．63％、14．29％。结论 妊娠晚期合并重型肝炎应早期诊断，早期采用综合治疗方案，及时终止妊娠，预防和减少各种并发症，是提高成活率的有效措施。

Retrospective analysis of 55 patients with severe hepatitis during late period of pregnancy

Objective To study the relationship between survival rate and pathogen,subtype,stage,delivered methods of 55 patients with severe hepatitis during late period of pregnancy.Methods Clinical data of 55 cases with severe hepatitis during late period of pregnancy was analyzed retrospectively.Results The incidence of hepatitis B was highest in patients with severe hepatitis during late period of pregnancy (84.5%).In subtypes,the incidence rates of chronic severe hepatitis,acute severe hepatitis and subacute severe hepatitis were 83.6%,5.5%,10.9% respectively(P<0.01).The survival rate in acute and subacute severe hepatitis was significantly higher than that in chronic severe hepatitis(P<0.01).In delivered methods,The survival rate of cesarean section was higher than that of aginal deliveries.The survival rate in early,middle and late stage of severe hepatitis were 90%,63.63%,14.29% respectively.Conclusions In the patients with severe hepatitis during late period of pregnancy,early diagnosis,multiple treatment as early as possible,pregnancy-terminating on time,preventing and reducing the complications are the effective methods in improving survival rates.
