卒中单元模式下综合治疗脑卒中吞咽障碍效果评估

目的：探讨卒中单元模式下综合治疗脑卒中后吞咽障碍的效果。方法：将在我院神经科卒中单元病房及普通病房住院,洼田氏饮水试验评价在Ⅲ级及以上的脑卒中患者作为研究对象,普通病房组患者按照《中国脑血管病的防治指南》进行处理及常规基础护理,卒中单元组患者在此基础上采用综合治疗。治疗3～4周后比较二组吞咽障碍的疗效、肺部感染发生率及近期预后。结果：卒中单元模式下综合治疗吞咽障碍,痊愈24例,有效90例,无效26例,总有效率为81.4%,明显高于普通病房组（P〈0.05）。卒中单元组肺部感染43例,感染率为30.7%,而普通病房组肺部感染72例,感染率为50.7%,二组比较差异有显著性（P〈0.05）。治疗3～4周后卒中单元组患者NIHSS评分明显降低,BI明显增高,与普通病房组患者比较差异有显著性（P〈0.05）,近期预后好。结论：卒中单元模式下综合治疗脑卒中后吞咽障碍效果明显,肺部感染率低,近期预后好。

Evaluation of the effect of combined therapy on dysphagia of cerebral infarction patients in stroke unit

Objective：To assess the effect of combined theray on dysphagia of cerebral infarction patients in stroke unit.Methods： Cerebral infarction patients with dysphagia were divided into stroke unit group and general ward group.The patients in stroke unit group received standard therapy and swallowing function training,and those in the the general ward group received ordinary treatment.The swallowing function,pulmonary infection,Barthel Index（BI） and the National Institute of Health Stroke Scale（NIHSS） of the two groups were evaluated before and after 3-4 weeks＇ treatment. Results： The effective rate of dysphagia in stroke unit group was 81.4%, which was higher than that in the general ward group （P〈0.05）. NIHSS and the rate of pulmonary infection in the stroke unit group was lower than that in general ward group （P〈0.05）,but BI was higher （P〈0.05）. Conclusion： Combined theray on dysphagia of cerebral infarction patients in stroke unit can significantly improve swallowing function and reduce the rate of pulmonary infection and neurological defects.
