急性肺栓塞猪模型的肺动脉造影与病理对照

摘    要:目的探讨数字减影肺动脉造影对猪急性肺段动脉肺栓塞的诊断价值.方法采用13只中国实验用小型猪,经颈静脉途径向肺动脉注射明胶海绵栓子(直径3.8~4.2 )制作肺栓塞模型,栓塞前后行数字减影肺动脉造影,并以病理解剖为标准,探讨其敏感性、特异性.结果对195个肺段进行分析,其中有病理诊断阳性肺段46个,病理诊断阴性肺段149个,数字减影肺动脉造影阳性肺动脉段47个,其中出现2个假阳性.敏感性为98%,特异性为99%,诊断准确率98%.结论数字减影肺动脉造影对亚肺段水平肺动脉栓塞的敏感性和特异性很高.

Abstract:Objective To study the diagnostic value of digital subtraction pulmonary angiography in assessing acute segmental pulmonary embolism of swines. Methods Acute pulmonary embolism model was made in 13 Chinese mini-swines by injecting glutin embolus into pulmonary artery via internal jugular. Digital subtraction pulmonary angiography was performed pre-creation and post-creation of the acute pulmonary embolism model. The sensitivity and specificity of the digital subtraction pulmonary angiography were evaluated on the basis of pathologic criterion. Results Total 46 positive and 149 negative were proved in segmental pulmonary arteries on pathological study. Forty-seven segmental pulmonary arteries were displayed as abnormal in digital subtraction pulmonary angiography, with sensitivity 98% and specificity 99%, respectively. Conclusion The sensitivity and specificity of digital subtraction pulmonary angiography in diagnosing the sub-segmental pulmonary embolism are high.
