布地奈德雾化吸入对肺炎支原体肺炎患儿肺功能及体液免疫功能的影响

摘    要:目的 探讨肺炎支原体肺炎患儿(MPP)布地奈德雾化吸入治疗后的临床症状及患儿肺功能及体液免疫功能的变化。方法 选择黄山首康医院2014年5月至2016年5月诊治的220例MPP患儿,随机分为对照组和治疗组,每组110例。对照组给予常规治疗,治疗组在对照组的基础上给予布地奈德雾化吸入。比较两组患儿治疗前和治疗14 d后的临床疗效、肺功能及体液免疫功能的变化。结果 治疗组患儿的治疗总有效率为95.5%,对照组治疗总有效率为84.5%,两组差异有统计学意义(P<0.05);治疗组患儿的肺活量、达峰时间比、呼出75%潮气量时的瞬间流速与潮气呼气峰流速比明显优于对照组,治疗组患儿血清IgA、IgM和IgG均低于对照组,差异均有统计学意义(P<0.05)。结论 布地奈德雾化吸入可以明显改善MPP患儿的临床症状,有效促进患儿的肺功能恢复,并积极调节患儿的体液免疫,且无明显的安全性问题,值得应用推广。

Abstract:Objective To investigate the clinical symptoms and changes of lung function and humoral immune function after aerosol inhalation of budesonide in children with mycoplasma pneumoniae pneumonia. Methods A total of 220 cases of children with mycoplasma pneumonia treated in our hospital were randomly divided into control group and treatment group, with 110 cases in each group. The control group was treated with routine therapy. Based on the control group, the treatment group was given inhaled budesonide inhalation. The clinical efficacy and the time of disappearance of clinical symptoms, lung function and humoral immune function were compared between the two groups before and after treatment. Results The total effective rate of the treatment group was 95.5%, which was significant higher than that of the control group(84.5%), and the difference was statistically significant (P<0.05). The lung function index, time to peak and at the time of exhaled 75% tidal volume and the tidal peak expiratory flow velocity ratio and other indicators of the treatment group were significantly better than those of the control group. The levels of serum IgA, IgM and IgG of the treatment group were significantly lower than those in the control group, and the difference was statistically significant (P<0.05). Conclusion Budesonide aerosol inhalation can significantly improve the clinical symptoms of children with mycoplasma pneumoniae pneumonia,effectively promote the recovery of lung function in children, and actively regulate children''s humoral immunity,without significant security issues, thus it is worth promotingin clinics.
