Analysis of lymphocyte subsets,immunoglobulins and complements in the patients with hepatitis B infection

Objective:To study the expression of lymphocyte subsets,immunoglobulin and complement in the peripheral blood of patients with chronic hepatitis B.Methods:132 cases of hepatitis B patients were selected as the research objects,from Baiyun Hospital Affiliated of Guizhou Medical University.According to the different levels of ALT in serum,they were divided into ALT normal group,ALT moderate group and ALT high group.According to the different levels of viral load,they were divided into non replication group,low replication group and high replication group.Another 30 healthy people were randomly selected as the control group.The contents of IgG,IgM,IgA,C3 and C4 in peripheral blood serum were determined by immunoturbidimetry.The absolute number of T,B and NK lymphocyte subsets in peripheral blood was determined by flow cytometry.Results:1.The expression of IgG,IgM and IgA in hepatitis B patients is as follows:①The levels of IgG,IgM and IgA in ALT high elevation group were significantly higher than that in the normal control group(P<0.05);②The levels of IgG increased in ALT normal group and virus load group,and the levels of IgM increased in ALT medium and non replication group(P<0.05),which was statistically significant;The levels of IgA increased in ALT medium and high elevation groups(P<0.05).2.The levels of complement C3 and C4 in patients with hepatitis B were significantly lower than those in normal control group(P<0.05);The level of C4 in normal ALT group was significantly lower than that in high ALT group(P<0.05).3.The number of CD3⁺CD4⁺T,CD3⁺CD8⁺T,CD19⁺B and CD16⁺/CD56⁺NK in each group were compared as follows:①Among different levels of ALT group,CD16⁺/CD56⁺NK decreased in each group;CD3⁺CD4⁺T decreased only in normal group;CD3⁺CD8⁺T decreased in normal group and high elevation group(P<0.05).In the viral load groups,CD3⁺CD8⁺T and CD16⁺/CD56⁺NK decreased;CD3⁺CD4⁺T and CD19⁺B decreased only in the non replication group and low replication group(P<0.05).②The levels of CD3⁺CD4⁺T,CD3⁺CD8⁺T,CD19⁺B,CD16⁺/CD56⁺NK and CD3⁺CD4⁺T in moderate elevation group were significantly higher than those in normal ALT group(P<0.05);③CD3⁺CD4⁺T and CD3⁺CD8⁺T in high replication group were significantly higher than those in non replication group(P<0.05).Conclusion:The immune system of patients with chronic hepatitis B is disordered,which is mainly manifested as hyperfunction of humoral immunity and inhibition of cellular immunity.The analysis of lymphocyte subsets,immunoglobulin and complement levels has certain significance for the clinical diagnosis,treatment and prognosis of chronic hepatitis B.
