Abstract:Objective To explore the relationship between cognitive impairment caused by cerebral small vessel disease(SVD)and the level of serum high mobility group protein B1(HMGB1)in patients.Methods A total of168 patients with SVD treated in our hospital from April 2019 to April 2021 were selected.According to the results of cognitive function evaluation,the patients were divided into normal cognitive group(n=94)and cognitive impairment group(n=74).The general data and laboratory inspection indicators of the two groups were compared;the factors affecting cognitive impairment were analyzed by multivariate Logistic regression analysis;the correlation between HMGB1 and TC,UA,Lpa,FIB,Dmure D,Hcy,hs-CRP was analyzed by Pearson correlation test;the predictive value of HMGB1 in cognitive impairment in patients with SVD was evaluated by ROC curve.According to the critical value of HMGB1,the patients were divided into low risk group and high risk group.The survival curve was drawn by KaplanMeier method to compare the cognitive status of the two groups.Results Age>65,junior high school education,smoking history,cerebral infarction,hypertension,hyperlipidemia,LI combined with WMLs,TC,UA,Lpa,FIB,Dmurd,Hcy,hs-CRP,HMGB1 were all factors leading to cognitive impairment in patients with SVD.HMGB1 was positively correlated with TC,UA,Lpa,FIB,DLV D,Hcy and hs-CRP,respectively.The area under the ROC curve of HMGB1 for predicting cognitive impairment in SVD patients was 0.841(95%CI:0.785-0.897,P<0.001),the best critical value was 6.74μg Universe L,the sensitivity was 92.46%,and the specificity was 88.37%.The incidence of cognitive impairment in the high-risk group(76.54%)was significantly higher than that in the low-risk group(13.79%).Conclusion HMGB1 is related to cognitive impairment in patients with SVD and has high predictive value.It can be used to identify high-risk groups in clinic.It is beneficial to take positive and effective treatment measures to reduce brain lesions,delay the progression of SVD and prevent the occurrence of cognitive impairment.
