不同剂量甲泼尼龙治疗重症手足口病疗效观察

目的观察使用不同剂量的甲泼尼龙对重症手足口病的治疗效果。方法将120例患有重症手足口的患儿随机分为大剂量组(60例)和小剂量组(60例),两组患儿在使用丙种球蛋白的基础上,分别联合使用大剂量甲泼尼龙(10～20 mg)和小剂量甲泼尼龙(1～2 mg)进行治疗。治疗后对两组的机械通气时间、住院时间、肺出血、肺水肿、休克发生情况以及好转情况进行比较。结果大剂量组住院时间明显长于小剂量组(P<0.05);大剂量组肺出血、肺水肿、休克发生率明显高于小剂量组(P<0.01),好转率低于小剂量组(P<0.05)。结论应用大剂量的甲泼尼龙对重症手足口病的患儿进行治疗,增加了患儿的肺出血、肺水肿以及休克的发生率,联合应用小剂量的甲泼尼龙进行治疗患儿临床症状明显改善。

Curative Effect Observation on Different Doses of Methylprednisolone in the Treatment of Severe Hand-foot-mouth Disease

Objective To observe the effect of different doses of methylprednisolone for severe hand foot mouth disease(HFMD).Methods 120 patients suffering severe HFMD were randomly divided into big-dose group of 60 cases and small-dose group of 60 cases,which were given big dose methylprednisolone(10-20 mg)and small dose methylprednisolone(1-2 mg)on the basis of Gamma globulin respectively.After treatment,mechanical ventilation time,hospitalization time,pulmonary hemorrhage,pulmonary edema,shock and improvement of the two groups were compared.Results The hospitalization time of the big-dose group was significantly longer than the small-dose group(P<0.05);pulmonary hemorrhage,shock,pulmonary edema of the big-dose group were significantly higher than the low-dose group(P<0.01),improvement rate was lower than the low-dose group(P<0.05).Conclusion The application of big dose of methylprednisolone for severe HFMD in children increases the incidence of pulmonary hemorrhage,pulmonary edema and shock,while combined application of small dose of methylprednisolone therapy in children has no obvious clinical symptoms improvement.
