食管癌组织中p53蛋白的表达及临床意义

目的　探讨p5 3基因突变和p5 3蛋白表达与食管癌临床病理的关系。方法　应用银染PCR -SS CP方法和免疫组织化学方法检测了食管癌组织中p5 3基因突变和蛋白的表达。以切缘正常组织为对照。结果　 4 7例食管癌标本中 ,p5 3基因第 5～ 8外显子出现点突变者 2 0例 ,其突变率为 4 2 .6 % ,生存期大于 5年组和生存期小于 5年组 ,p5 3突变率分别为 32 .1%和 5 7.9% ,差异无显著性 (χ2 =3.0 71,P >0 .0 5 ) ;2 2例伴有淋巴结转移的原发癌中 13例出现了突变 ,阳性率为 5 9.1% ,与无淋巴结转移组相比 ,有显著性差异 (P <0 .0 5 )。p5 3蛋白表达者 2 9例 ,其表达率为 6 1.7% ,生存期大于 5年组与生存期小于 5年组相比 ,p5 3蛋白阳性表达率分别为 5 0 .0 %和 78.9% ,具有显著性差异 (P <0 .0 5 ) ,有淋巴结转移组和无淋巴结转移组相比 ,p5 3蛋白表达阳性率分别为 77.3%和 4 8.0 % ,也具有显著性差异 (P <0 .0 5 )。结论　p5 3基因突变和蛋白表达与食管癌的转移密切相关 ,且是一个判断预后的有用指标

p53 Protein Expression and Its Significance in Human Esophageal Carcinorma

Objective To determine clinical significance and expression of p53 protein in human esophageal carcinorma tissue.Methods To measure p53 protein expression in 47 cases esophageal carcinorma tissue by immunohistochemical and PCR-SSCP method, the adjacent normal tissues of the same cases were used as control. Results p53 of 13 cases of aesophageal carcinorma tissue of lymphnode metastasis group (22 cases) had been mutated, the positive rate (59.1%) was significantly different from the non-lymphnode metastasis group(P<0.05).29 cases had been expressed p53 protein, the protein expression rate was 61.7%; there was significant difference between the survived and the dead esophageal carcinorma patients 5 years after operation, the p53 protein expression rate was 50.0% and 78.9% respectively, there was significant difference between the lymphnode metastasis group and the non-lymphnode metastasis group (the p53 protein expression rate was 77.3% and 48.0%). Conclusion The p53 mutation and protein expression are related to lymphnode metastasis of aesophageal carcinorma,and can be an important maker of identifying the prognosis of esophageal carcinorma.
