血浆ET和CGRP对先心病肺动脉高压术后的调控

摘    要:目的 :探讨血浆内皮素 (ET)、降钙素基因相关肽 (CGRP)在先天性心脏病伴肺动脉高压围术期的调控作用。方法 :采用放射免疫法动态测定 组 (轻度肺高压 )、 组 (中重度肺高压 )共 33例左向右分流先心病伴肺高压患儿血浆 ET、CGRP值。结果 :血浆 ET术前两组均增高 ,且 组明显高于 组 (138± 2 5 vs 77± 19) pg/ ml,P<0 .0 1],体外循环 (CPB)术后 组较术前升高 (P<0 .0 0 1) ,1d后下降到术前水平 (P>0 .0 5 ) , 组 3d后才呈现下降 (P<0 .0 5 ) ;血浆 CGRP 、 组术后 1d显著升高 (187± 131、330± 187) pg/ ml],术后 3d下降 (88± 85、15 0±95 ) pg/ ml]。结论 :血浆 ET和 CGRP参与了先心病肺高压的形成过程 ,并与肺高压的程度有一定关系。CPB术后血浆 ET和 CGRP的失衡可能影响手术预后。

Abstract:OBJECTIVE: To assess the perioperative changes in plasma endothelin (ET) and calcitonin gene-related peptide (CGRP) in children with congenital heart disease complicated by pulmonary hypertension. METHODS: In 33 patients, radioimmunoassay was used to measure plasma ET and CGRP levels at different time points: Preoperative, immediate post-op, 3 hrs, 24 hrs, 72 hrs after cardiac surgery with cardiopulmonary bypass. RESULTS: Preoperative plasma ET levels in two groups were markedly increased. The level of ET in group II (severe pulmonary hypertension patients) was significantly higher than those (138+/-25 compared with 77+/-19)pg/ml, P<0.01] in group I (mild pulmonary hypertension patients). In group I, the plasma ET level rose immediately post-op (P<0.001) and retumed to the initial level after one day (P>0.05). However, in group II plasma ET level was only normalized 3 days post-op (P<0.05). The CGRP level of both group I and group II increased at the first post-op day (187+/-131 330+/-187)pg/ml], then began to decline by the 3rd day (88+/-85, 150+/-95) pg/ml]. CONCLUSION: Endothelin and calcitonin gene-related peptide was involved in the pathophysiologic process of pulmonary hypertension and correlated with the degree of pulmonary hypertension. The imbalance of ET and CGRP after cardiopulmonary bypass may affect the prognosis.
