肺癌患者血清NSE,CEA及SCCAg检测的临床意义

对１４１例肺癌患者血清ＮＳＥ、ＣＥＡ及ＳＣＣＡｇ进行检测，血清ＮＳＥ在小细胞肺癌（ＳＣＬＣ）具有较高的阳性率和组织学特异性；血清ＳＣＣＡｇ的升高以非小细胞肺癌（ＮＳＣＬＣ）为主。具有较高的组织学特异性和阳性预测值（ＰＶ＋）（８６．２％），但阳性率较低（２６．６％）；而血清ＣＥＡ在两型肺癌中均有升高，但阳性诊断率不高（ＳＣＬＣ：４０．４％，ＮＳＣＬＣ：２６．６％），３项指标联合检测可不同程度地提高肺

Clinical evaluation of serum NSE,CEA and SCCAg in lung cancer

A retrospective investigation of serum NSE,CEA and SCCAg was made on 141 serum samplesfrom patients with lung cancer,including 47 cases with SCLC and 94 with NSCLC. In SCLC,serum NSEwas shown to be higher in sensitivity (59.4%)and tissue specificity(79.8%).In NSCLC,serum SCCAgpossesed higher tissue specificity(91.5%)and positive predictive value(PV )(86.2%),but its diagnosticsensitivity was lower(26.6%).Serum CEA as tumor marker was low in sensitivity(40.4% in SCLC,26.6%in NSCLC).The combination of these three tests provided advantage for the recognition of lung can-cer.The levels of serum NSE,CEA and SCCAg were more frequently elevated in patients with disseminat-ed disease than in that with localized disease,It was concltided that serum NSE was of clinical significancein the diagnosis of SCLC,whereas the clinical values of CEA and SCCAg probably lie mainly in monitoringthe course of lung cancer.
