喉癌细胞外基质糖蛋白的表达

摘    要:目的 :探讨细胞外基质糖蛋白 (tenascin ,TN)在喉部良、恶性病变中的表达 ,及其与喉肿瘤发生及发展的关系。方法 :将 5 8例石蜡包埋喉部病变组织用鼠抗人单克隆抗 TN抗体行免疫组化染色 ,用HPIAS 10 0 0型医学彩色全自动图像分析系统进行分析。结果 :TN在喉部良性病变及正常组织的细胞外间质中无阳性染色 ,在喉的原位癌组织的细胞外间质中有弱阳性染色 ,且二者差异有显著性意义 (P <0 .0 0 1)。在喉的侵袭性鳞状细胞癌组织的细胞外间质中有强的TN染色 ,且在有淋巴结转移的肿瘤组织中染色更强 ,染色面积更广。在喉癌的各病理分级之间阳性染色差异无显著性意义 (P >0 .0 5 )。结论 :提示TN与喉癌的发生、发展和转移有关 ,而与其病理分级无关。

Abstract:OBJECTIVE: Tenascin (TN) is an extracellular matrix glycoprotein upregulated in some pathological processes. In the study, we investigated its expression in the laryngeal epithelial lesions and explored the relation of TN with the laryngeal tumor. METHOD: Expression of TN were examined by immunohistochemistry SP method in sections of laryngeal lesions from 4 leukoplakia, 8 papilloma, 7 in situ carcinoma and 39 invasive squamous cell cancer patients. An automatic image analyzer was used to analyze the staining results. RESULT: No expression, of TN was found in the extracellular matrix of papilloma or leukoplakia. Only a weak stain was found at their epithelialstromal interface in seldom sections. There was a weak extracellular matrix stain of TN in all situ carcinoma and the stain difference between in situ cancer and papilloma or leukoplakia was significant (P < 0.001). There was a much stronger extracellular matrix stain in invasive cancer than that in situ cancer (P < 0.01). The differences of TN expression among different T stage and histological grade of invasive cancer were insignificant (P > 0.05). But there was a significantly stronger TN stain in the cases with lymph node metastasis than those without lymph node metastasis (P < 0.01). CONCLUSION: TN expression may be involved in the oncogenesis and metastasis of laryngeal squamous cell cancer. It was implicated that the risk of pre cancerous lesions progress to cancer.
