同型半胱氨酸、超敏C反应蛋白与冠心病关系的研究

摘    要:目的 探讨血清同型半胱氨酸(Hcy)、超敏c反应蛋白(hs—CRP)与冠心痛(CHD)的关系。方法收集本院169例冠心病患者血清为观察组,其中稳定性心绞痛42例(SAP组),不稳定性心绞痛62例(UAP组),急性心肌梗死65例(AMI组),以62例健康人血清为对照组,测定Hcy、hs—CRP的水平。结果CHD患者Hcy和hs—CRP水平与对照组相比显著增高,SAP、UAP、AMI3组Hey比较,水平依次升高,且差异有统计学意义(P〈0.05);AMI组、UAP组和SAP组hs—CRP水平相比,差异有统计学意义(P〈0.05),但AMI纽和UAP组两组间差异无统计学意义(P〉0.05)。Hcy单独检测的CHD诊断敏感度为64.12%,特异度56.46%;hs—CRP单独检测敏感性为70.42%,特异度为44.76%;两者联合检测敏感度为88.05%,特异度为80.64%,明显高于单独检测(P〈0.05)。结论Hcy、hs—CRP是冠心病的独立危险因素,两者同时检测对可冠心病的诊断的重要价值。

Abstract:Objective To study the correlation between serum homocystein (Hcy), hyper sensitive C - reactive protein (hs - CRP) and coronary heart disease (CHD). Methods 169 CHD patients were divided into 3 groups, including 42 eases of stable angina (SAP), 62 cases of unstable angina ( UAP), 65 cases of acute myocardial infarction (AMI), and control group consisted of 62 healthy people. The level of hs - CRP and Hcy was measured. Results The levels of Hcy and hs - CRP were significantly higher in CHD patients than those in control group ( P 〈 0. 05 ). Compared with SAP group, the levels of/Icy and hs - CRP were increased in AMI and UAP groups. The level of hs - CRP was posi- tively associated with the level of Hcy (P 〈 0.05 ). The difference of AMI and UAP groups is not statistically significant. In addition, the sensitivity of single detection of Hcy was 64.12 %, the specificity was 56.46% ; the sensitivity of single detection of hs - CRP was 70.42 %, the specificity was 44.76% ;While the joint detection sensitivity (88.05)and the specificity(80.64) was higher than the single detection. Conclusions Hcy and hs - CRP is the independent risk factors of CHD,the joint detection of Hcy and hs - CRP plays an important role in dignosis of CHD.
