胰腺癌患者外周血MUC4、MMP-9mRNA的表达及临床意义

目的研究MUC4mRNA和MMP-9mRNA在胰腺癌外周血单个核细胞上的表达,探讨其在胰腺癌早期诊断、鉴别诊断、转移监测及预后评价中临床价值。方法采用荧光定量RT—PCR技术检测胰腺癌、胰腺良性疾病和健康人外周血单个核细胞上的MUCAmRNA和MMP-9mRNA的表达,同时检测血清CA19—9、CA242水平,分析MUC4mRNA、MMP-9mRNA与CA19—9、CA242之间相关性。结果胰腺癌患者外周血中MUC4mRNA的阳性率为64．3％,特异性100％;胰腺良性疾病及健康对照组中未见MUC4mRNA的表达;MMP-9mRNA的阳性率为69．6％,诊断特异性85％,胰腺癌MUCAmRNA和MMP-9mRNA的表达明显高于胰腺良性疾病组和健康对照组：P〈0．01）,且二者水平变化呈正相关。MUCAmRNA和MMP-9mRNA阳性表达率随胰腺癌临床分期的增加及淋巴转移而逐渐增高。结论MUC4和MMP-9是胰腺癌早期诊断和鉴别诊断的良好指标,肿瘤标志物联合检测,可有效提高胰腺癌早期诊断灵敏度和特异性。

The expression and clinical significance of MUC4 and MMP - 9mRNA in peripheral blood of pancreatic cancer patients

Objective To explore the expression and clinical significance of MUC4 mRNA and MMP - 9 mRNA in peripheral blood mononuelear cells of pancreatic cancer patients. To research the early diagnosis, differential diagnosis, transfer monitoring and prognostic clinical value of pancreatic cancer. Methods The expression of MUCA mRNA and MMP - 9 mRNA in peripheral blood, which were collected from pancreatic cancer and benign pancreatic disease and healthy human, were detected with reverse transcription realtime PCR. At the same time, the concentration of CA19 - 9, CA 242 were measured, then the relevance betweentwo new protein markers of MUCA mRNA, MMP- 9 mRNA and CAI9 - 9, CA242 was compared, and study the value in the diagnosis of pancreatic cancer. Results The MUC4 mRNA - positve rote of is 64.3% , and specificity is 100% , there was no MUCAmRNA expression in benign pancreatic disease and healthy controls, the MMP- 9 mR- NA - positive rate is 69.6%, and specificity is 85% . The expression of MUC4 mRNA and MMP - 9 mRNA in pancreatic cancer is higher than in benign pancreatic disease and hea]thy human（ P 〈 0.01 ）, MUCA mRNA and MMP - 9 mRNA positive expression rate gradually increased with the increase of the pancreatic cancer clinical stage and lymph node metastasis. Conclusions The MUCA and MMP - 9 are good indlcators of early diagnosis and differential diagnosis of pancreatic cancer. Joint detection Can effectively improve the sensitivity and specificity of early pancreatic cancer diagnosis.
