腰椎间盘突出合并侧隐窝狭窄症(附43例报告)

本文报告腰椎问盘突出合并侧隐窝狭窄症43例.主要采取半椎板切除、摘除髓核加侧隐窝扩大减压术.优良率94.6%.手术的关健是切除侧隐窝关节突内侧肥厚的黄韧带及椎体后缘骨赘,注意神经根管受压,要彻底减压.通过病史及手术所见,我们认为其发病是先有椎间盘突出,而后继发侧隐窝狭窄.故狭窄基本上是继发于椎间盘退变和突出.

Prolapse of Lumbar Intervertebral Disc with Lateral Recess Stenosis

Reported are 43 cases of prolapse of lumbar intervertebral disc with lateral recess stenosis treated mainly by laminectomy,removal of nucleus pulposus and decompression of lateral recess.The effec- tive rate is 97.6%.The crux of the operation is to resect the hypertrophic yellow ligament medial to the articular process of the lateral recess and the osteophyte in the posterior edge of lumbar vertebral body.Be sure to decompress nerve root tube thoroughly.On the basis of medical history and what is seen in the operations.We believe that prolapse of lumbar intervertebral disc developed in advance of lateral recess stenosis and,the stenosis is secondary to retrograde degeneration and prolapse of lumbar intervertebral disc.
