综合医院内科住院病人焦虑抑郁状况研究

目的了解综合医院内科住院病人焦虑和抑郁情绪状况。方法应用焦虑自评量表(SAS)和抑郁自评量表(SDS),对华北煤炭医学院附属医院内科不同科室200名病人进行抽样测试研究。结果1在176例被调查者中,单纯焦虑者7例(4.0%),单纯抑郁者10例(5.6%),焦虑合并抑郁者39例(22.0%)。总计发生焦虑者46例(26%),发生抑郁者49例(27.7%)。2内科病人的SAS评分显著高于全国常模(t=7.586,P0.001),而SDS评分与全国常模间无显著性差异(t=0.008,P0.05);3不同居住地、工作情况的病人焦虑情况无显著性差异(t=1.547,P0.05),但抑郁情况有显著性差异(F=1.972,P0.05);4不同婚姻状况、家庭和谐状况的病人焦虑抑郁情况均有非常显著的差异(F=6.381,6.340,P0.001);5不同文化程度的病人焦虑抑郁情况均有显著性差异(F=5.331,5.060,P0.01);6不同科室的病人焦虑抑郁情况均有非常显著的差异(F=12.313,14.40,P0.001),进一步进行多重比较表明,呼吸内科病人SAS、SDS得分明显高于其他内科病人。结论内科病人的焦虑情绪受婚姻状况、家庭和谐情况影响,抑郁情绪受居住地、工作情况、婚姻状况、家庭和谐情况、文化程度因素影响;呼吸内科病人焦虑抑郁程度显著高于其他内科病人。

Anxiety and Depression of Patients of Medical Department in Polyclinic

Objective To explore the anxiety and depression condition of patients of medical department in Polyclinic. Methods A total of 200 medical patients of the hospital attached to North China Coal Medical University were tested with Self-rating Anxiety Scale （SAS） and Self-rating Depression Scale （SDS）. Results ①In all of the 176 subjects,the proportion of pure anxiety,pure depression and anxiety with depression were 4.0%,5.6% and 22.0%. The proportion of anxiety and depression were 26.0% and 27.7%. ②The average score of SAS of medical patients were significantly higher than those of the common people （t=7.586,P〈0.001）,but there were no significant difference on the average score of SDS between medical patients and common people （t=0.008,P〉0.05）. ③There were no significant difference of the anxiety condition between urban and rural patients,patients who had jobs and had no jobs （t=-1.574,P0.05）,but it was obvious on the depression condition （t=-1.972,P0.05）. ④There were significant difference of the anxiety and depression condition between patients whose family was harmonious and was not harmonious,also it was obvious among patients who had different marriage conditions （F=6.381,6.340,P0.001）. ⑤There were significant difference of the anxiety and depression condition between patients who had different educated degree （F=5.331,5.060,P0.01）. ⑥There were significant difference of the anxiety and depression condition among patients of different medical departments （F=12.331,14.40,P0.001）. After doing multiple comparison,we found that the average scores of SAS and SDS of patients in respiratory medicine were significantly higher than those of other medical departments. Conclusion The anxiety degree of medical patients were significantly higher than those of the common people,but the depression degree were identical with those of the common people; Marital status,harmony of family and educated degree were influence factors of anxiety condition of medical department patients,the place of residence,working situation,marital status,harmony of family and educated degrees were influence factors of depression condition; the anxiety and depression degree of patients in respiratory medicine were significantly higher than those of other medical departments.
