摘    要:目的 观察新生儿缺氧缺血性脑病(hypoxic-ischemic encephalopathy,HIE)星形胶质细胞的变化及其与临床的关系.方法 2002年1月至2005年12月通过对吉林大学第一医院25例新生儿HIE患儿脑病理标本进行免疫组化观察GFAP标记的星形胶质细胞的变化,进一步综合分析其与新生儿HIE的发生、病程的关系.结果 25例新生儿HIE脑标本均可见不同程度的星形胶质细胞增生,小脑星形胶质细胞表现为生后24h内死亡者增生程度重(P<0.05),生存时间越长,增生程度反而较轻;慢性缺氧时大脑星形胶质细胞增生程度重(P<0.05),急性缺氧时延髓增生程度重(P<0.05),混合性缺氧大脑及延髓增生程度均较重(P<0.05).结论 新生儿HIE星形胶质细胞增生程度与HIE的发生、病程密切相关.

Studies on the relationship between astrocyte and clinical appearance in neonatal hypoxic-ischemic encephalopathy
Abstract:??Objective To observe the relationship between the change of astrocyte and clinical appearance in neonatal hypoxic??ischemia encephalopathy (HIE).Methods The pathological brain specimens of 25 newborn who died of HIE were observed macroscopically and microscopically.The immunochemistry staining with GFAP was used to observe the change of astrocyte.The relationship between astrocyte and the occurrence of HIE,course of disease and newborn degree of maturity were further synthetically analysed.Results The brain specimens of 25 newborn HIE have different??degreed astrocyte hyperplasia,Cerebellar hyperplasia show that hyperplasia is severe in the cases who dead within 24 hours(P<0.05),the longer survived time the less hyperplasia it showed.Cerebral and medulla oblongata astrocyte hyperplasia is independent of survival time.Cerebral astrocyte hyperplasia in the chronic incomplete hypoxia cases are severe(P<0.05); the change of medulla oblongata in the acute complete hypoxia cases are severe(P<0.05).the change of cerebral cortex and medulla oblongata are all severe in the cases of mixed hypoxia(P<0.05).Conclusion The degree of astrocyte hyperplasi are closely related to the occurrence and course of disease in HIE.
