老年前列腺增生合并糖尿病开放性手术的围手术期处理

目的　探讨老年前列腺增生合并糖尿病的特点及围手术期的处理原则。方法　对 5 6例老年前列腺增生合并糖尿病的患者围手术期临床表现、诊断与治疗进行回顾性分析。结果　糖尿病以 2型多见 ,本组 5 3例 (94 6 % ) ;DM可产生尿潴留等与BPH相似的症状 ,影响手术指征及预后 ,本组术后尿潴留未改善 3例 (5 4 % ) ;尿路和切口感染率较高 ,本组术后尿路感染 3例 (5 4 % ) ,切口感染 4例 (7 1% )。结论　围手术期的处理重点是有效控制血糖、维持机体内环境的稳定、预防和控制感染 ,术前最好做尿动力学检查 ,以正确判定手术指征 ,提高治愈率

Perioperative management of in the elderly patients with hyperplasia of prostate and diabetes mellitus

Objective To discuss the characteristics and perioperative management of in the elderly patients with hyperplsia of prostate and diabetes.Methods 56 elderly cases with hyperlasia prostate and diabetes were retrospectively analyzed.Results The associated diabetes presented frequently in type Ⅱ.There were some semblable symptoms in BPH and DM,such as retention of urine.Therefore,BPH operation indication may be interfered,expectant effect of operation may be affected.Retention of urine remained in 3 cases after operation.Infection rate of urinary tract and incision was higher.After operation,3 cases had infection of urinary tract,4 cases infection of incision.Conclusion The keys of perioperative management are blood sugar should be controlled;the stabilization of the body's homeostasis maintained;the infection prevented and treated effectively;the operative indication utilized correctly,which all play important roles on prognosis of the patients.
