大承气汤加味治疗粘连性肠梗阻疗效观察

目的:观察大承气汤加味在肠梗阻治疗中的疗效。方法:将94例肠梗阻患者分治疗组51例,对照组43例。对照组予西医保守治疗;治疗组在西医治疗基础上予大承气汤加味胃管注入。结果:治疗组总有效率及痊愈率分别为90.2%和68.6%,明显高于对照组的74.4%和41.9%(P<0.05及P<0.01);无效转手术率治疗组明显低于对照组(9.8%vs 25.6%,P<0.01);肛门排气时间治疗组明显短于对照组(40.04±10.72)h vs(53.39±8.08)h,P<0.01;住院时间治疗组明显短于对照组(7.14±1.51)天v(s9.37±1.99)天,P<0.01;治疗72h后,外周血C-反应蛋白(CRP)治疗组明显低于对照组(P<0.01),白细胞计数(WBC)两组无差异(P>0.05)。结论:承气汤加味灌胃结合西医保守治疗在肠梗阻治疗中疗效显著,值得临床推广应用。

Treatment of Adhesive Intestinal Obstruction with Dachengqi Decoction

Objective:To observe the effect of Dachengqi decoction in the treatment of adhesive intestinal obstruction.Methods:94 cases were divided into 2 groups,the control group(43 cases)was treated by conservative treatment and treatment group(51 cases)was treated by routine western medicine conbined with Dachengqi decoction.Results:The total effective rate and cure rate was 90.2% and 68.6% in treatment group,significantly higher than that of control group 74.4% and 41.9%(P<0.05 and P<0.01);invalid operation rate of the treatment group was significantly lower than control group(9.8% vs 25.6%,P<0.01);anal exhaust time of the treatment group was significantly shorter than control group(40.04±10.72)h vs(53.39±8.08)h,P<0.01;Length of time in hospital,treatment group was shorter than control group(7.14±1.51)d vs(9.37±1.99)d,P<0.01;CRP in treatment group was significantly lower than that of the control group After 72 hours of treatment(P<0.01);no difference in WBC of two groups(P>0.05).Conclusion:There is significant effect in the treatment of intestinal obstruction by Dachengqi Decoction combined with WM,worthy of further clinical application.
