大隐静脉腔内射频闭合术治疗下肢慢性静脉功能不全

目的:探讨腔内射频闭合联合点式剥脱术治疗下肢慢性静脉功能不全的安全性和有效性.方法:将40例下肢慢性静脉功能不全患者随机分成两组:射频组(20例)行大隐静脉腔内射频闭合联合曲张静脉点式剥脱术,对照组(20例)行大隐静脉高位结扎抽剥术加曲张静脉点式剥脱术.比较两组疗效、手术时间、SF-36生活质量量表评分、疼痛评分、住院时间、住院费用以及并发症的发生等情况.结果:两组1年后疗效相当,射频组在手术时间、生活质量评分、疼痛评分、住院时间等方面有明显统计学优势;对照组在住院费用方面有明显统计学优势.结论:腔内射频闭合联合点式剥脱术治疗单纯大隐静脉慢性功能不全疗效确切,安全性好,患者可获得较高的生活质量和舒适度,但费用较昂贵.

Radiofrequency obliteration in treatment of chronic venous insufficiency of lower extremities

Objective: To evaluate the efficacy and safety of radiofrequency(RF) obliteration in treatment of chronic venous insufficiency of lower extremities(CVI) in comparison with ligation plus vein stripping.Methods: Forty patients were randomized into two groups: RF group of 20 that were given endovenous RF obliteration of GSVs and phlebectomy of varicose vein,and control group of 20 that were given traditional high ligation plus vein stripping.Outcomes were evaluated between two groups,including medical outcomes,procedure time,short form 36 questionnaire(SF-36),pain scoring,hospitalization time,cost and complications.Results: All patients in two groups employed the procedures successfully.In 1 year follow-up,the rate of complete disappearance of varicosity in both groups was identical.RF group showed better results than the control group with procedure time,quality of life scoring,pain scoring,hospitalization time.But cost of hospitalization in RF group was significantly higher than the control group.Conclusion: Radiofrequency obliteration is an effective and safe method in treatment of chronic venous insufficiency of lower extremities,especially improving quality of life comparing to traditional ligation plus vein stripping,but the cost of RF obliteration is expensive.
