银屑病312例与代谢综合征相关性的临床分析

目的观察银屑病患者312例合并代谢综合征(MS)的患病率及血压、体重指数(BMI)等代谢指标,研究银屑病严重程度与MS的相关性。方法将红皮病型银屑病、泛发性脓疱型银屑病及关节病型银屑病合称为重型银屑病,寻常性银屑病为轻型银屑病,对312例轻型和重型银屑病患者的临床资料进行回顾性分析。结果 312例银屑病患者中MS的患病率为25.96%,轻型组为27.23%,重型组为23.97%。轻型组高血压患病率、体重、BMI及收缩压较重型组明显升高(P<0.05),两组间MS及其他组分患病率、各年龄段MS患病率差异无统计学意义(P>0.05)。重型组白细胞数、中性粒细胞百分比、血沉较轻型组升高,总蛋白、白蛋白、血尿素氮、血肌酐、血尿酸较轻型组低,差异均有统计学意义(P<0.05)。银屑病合并MS组的平均年龄高于非合并MS组(P<0.05),但两组的病程、发病年龄及不同类型银屑病的患病率差异无统计学意义(P>0.05)。结论银屑病严重程度与MS及各组分的患病率无显著相关性,对重型及轻型银屑病患者的MS及其组分均需及早干预和治疗。

Correlation Between Psoriasis and Metabolic Syndrome: Clinical Analysis of 312 Cases

Objective To investigate the clinical data of metabolic syndrome (MS), including blood pressure, body mass index (BMI) among 312 cases with psoriasis, and to investigate the correlation between severity of psoriasis and MS. MethodsErythrodermic psoriasis, generalized pustular psoriasis and psoriasis arthritis were defined as severe type of psoriasis, while psoriasis vulgaris were defined as common type. The clinical data of 312 cases with psoriasis were retrospectively reviewed. ResultsThe prevalence of MS in 312 cases with psoriasis was 25.96%, of which the common group and severe group were 27.23% and 23.97% respectively. The common group had a significantly higher prevalence of hypertension, weight, BMI and systolic blood pressure compared with the severe type (P<0.05). The prevalence of MS and the other MS components, MS of different ages had no statistical difference between the two groups (P>0.05). The severe group had significantly higher white blood cell count, neutrophil percentage and erythrocyte sedimentation rate than the common group, and the severe group had significantly lower levels of total protein, albumin, blood urea nitrogen, serum creatinine and uric acid than the common group (P<0.05). The age of psoriasis with MS cases were older compared with those without MS (P<0.05). However, there was no significant difference in the disease duration, onset age and the prevalence of different psoriasis types (P>0.05). ConclusionThere is no association between severity of psoriasis and prevalence of MS and its components. It is necessary to intervene and treat psoriasis with MS and its components early, whether the common type or severe type.
