微切口白内障超声乳化术的临床研究

摘    要:目的 探讨微切口白内障超声乳化术的临床疗效.方法 将90例白内障患者随机分为试验组44例(50只跟)和对照组46例(50只跟).试验组使用微切口白内障超声乳化术,对照组使用常规超声乳化术,观察两组术后视力、平均角膜内皮细胞密度、平均角膜散光度、平均手术源性散光度.结果 术后第1天、第7天及第30天的视力≥0.5比例分别是:试验组60%、72%、88%;对照组46%、62%、84%;术后第7天、第30天试验组平均角膜内皮细胞密度明显高于对照组,差异有显著性(P均<0.01);平均角膜散光度和平均手术源性散光度术后第7天和第30天对照组均大于试验组,差异有统计学意义(P<0.01).结论 双手微切口白内障超声乳化吸除术是一种新型的手术方法,具有安全、有效、微创地临床特点.

Clinical study of microphacoemulsification
Abstract:Objective To compare the clinical effect of the two different methods (micro-incision phacoemulsificatiun or regular phacomulsificaion) to treat the cataract patients.Methods Ninety cataract pa-tients were randomly divided into two groups: test group including 44 cases of 50 eyes, control group including 46 cases of 50 eyes.The test group was treated by micro-incision phacoemulsification, and the other was treated by regular phacomulsification.The vision and corneal endothelial cell density in both groups were observed pre-operatively, on the day 1, 7, and 30 postoperatively.Knife Keratometry measurements were obtained preoper-atively, at day 7 and 30 postoperative by Nikon Keratometry.The mean surgically induced astigmatism (SIA) and mean postoperative uncorrected visual acuity between the two groups were compared up to 6 months post-operatively, SIAs were analyzed by "Holladay vector method".Results A visual acuity of 0.5 or better in test group was obtained in 60%, 72% and 88% of the patients at day 1, 7 and 30 postoperatively.A visual acuity of 0.5 or better in control group was obtained in 46%, 62% and 84% of the patients at day 1, 7 and 30 post-operatively.There was a statistically significant difference between the two groups in mean endothelium cell count, surgically induced astigmatism and mean corneal astigmatism one week, one month after surgery.Con-clusions Micro-incision phaeoemulficatiun is feasible, safe and effective surgery methed for the cataract pa-tient.
