乌司他丁在体外循环期间的心肌保护作用

目的:观察体外循环(CPB)中预充乌司他丁对CPB心脏病患者血浆cTnI、CK、CK-MB的影响。方法:选择24例主动脉阻断时间在30min以上的心脏直视手术患者随机分为实验组和对照组各12例。实验组在常规CPB预充液中加入乌司他丁1.5万U/kg,对照组用生理盐水。分别在CPB前(T1)、开放升主动脉时(T2)、开放升主动脉30min(T3)、术毕12h(T4)、术毕24h(T5)、抽取桡动脉血,测定血浆的浓度。结果:实验组患者在T3、T4、T5血浆浓度明显低于对照组(P<0.01)。结果:乌司他丁可明显减少CPB术后的释放,对心肌缺血再灌注有保护作用。

Myocardial Protection of Ulinastatin in Cardiopulmonary Bypass in Patients.

Objective:To investigate influence of ulinastin on the release of concentration of CK,CK-MB,cTnI in plasma after cardiopulmonary bypass.Methods:24 patients scheduled for cardiac surgery under 30 minutes following CPB were randomly divided into two groups.In treated group(n=12),ulinastin was added into the prine before CPB,but in control group(n=12),saline was added.Blood samples were drawn from the radial artery at the following intervals:before CPB(T1),declamping of aorta(T2),30 minutes after declamping of aorta(T3),12 hours after CPB(T4),24 hours after CPB(T5).The concentration of CK,CK-MB,cTnI in plasma level were measured.Results:CK,CK-MB,cTnI in the treated groups were significantly lower than those in the control group at T3,T4,T5(P<0.01).Conclusion:Ulinastin can decrease the concentration of CK,CK-MB,cTnI in plasma after CPB and may alleviate heart muscle isehemia reperfusion injury during cardiopulmonary bypass.
