肠内营养在慢性阻塞性肺疾病机械通气患者中的应用

摘    要:目的:探讨肠内营养对慢性阻塞性肺疾病(COPD)机械通气患者营养状况改善效果。方法:60例慢性阻塞性肺疾病(COPD)机械通气患者随机分为两组,肠内营养组(EN组,n=30)和静脉营养组(PN组,n=30),观察两组患者在支持治疗前和治疗2周后血清总蛋白(TB)、血清白蛋白(ALB)、血红蛋白(Hb)、体重、肱三头肌皮褶厚度(TSF)及上臂肌围(MAMC)、机械通气时间和一次成功脱机率及并发症等。结果:EN组血清总蛋白、白蛋白、血红蛋白均较治疗前提高,患者体重、肱三头肌皮褶厚度及上臂肌围均增加(P<0.05);PN组以上指标治疗前、后比较差异无统计学意义(P>0.05);两组治疗后各项指标比较差异有统计学意义(P<0.05)。EN组呼吸机辅助机械通气时间较PN组缩短(P<0.05),EN组一次成功脱机率比PN组高(P<0.05)。EN组并发症发生率较PN组低(P<0.05)。结论:肠内营养可改善慢性阻塞性肺疾病机械通气患者营养状况。

Abstract:Objective:Explore enteral nutrition for chronic obstructive pulmonary disease(COPD) mechanical ventilation patients nutrition improve effect.Methods:60 patients with chronic obstructive pulmonary disease(COPD) mechanical ventilation patients were randomly divided into two groups: prealbumin(EN group,n=30) and intravenous fewer ten patients experienced septic complications(PN group,n= 30).Two groups of patients in support observation before treatment and treatment two weeks after serum total protein(TB),serum albumin(ALB),the hemoglobin(Hb),weight,humerus,triceps skinfold(TSF) and upper arm muscle wai(MAMC),mechanical ventilation time and a successful probability and take off complication.Results:EN group of serum total protein,albumin,hemoglobin which is improving before treatment,patients were dimension,humerus,triceps skinfold and the midarm muscle wai increased(P<0.05);above PN group index before and after treatment was not statistically significant comparative differences(P>0.05);the index of the two groups after treating comparing statistical significance(P<0.05).EN group breathing machine auxiliary mechanical ventilation time is shortened PN group(P<0.05),EN group a successful probability off than high PN group(P<0.05).EN group complications PN group is low(P<0.05).Conclusion:Enteral nutrition can improve chronic obstructive pulmonary disease patients with mechanical ventilation nutritional status.
